Abstract
Objective To observe the effects of Qingzao Jiufei Decoction on T lymphocyte subsets after thoracoscopic sublobar resection for non-small cell lung cancer(NSCLC).Methods A total of 141 patients who underwent thoracoscopic sublobar resection for NSCLC in the Department of Cardiothoracic Surgery of Affiliated Hospital of Nanjing University of Chinese Medicine from February 2020 to April 2023 were enrolled.According to postoperative treatment,patients were divided into a control group(n=72)and a treatment group(n=69).Propensity score matching was performed at a 1:1 ratio,and 58 patients were included in each group after matching.The control group received conventional treatment,while the treatment group received Qingzao Jiufei Decoction in addition to conventional treatment.Both groups were treated for 3 weeks.Pulmonary function,T lymphocyte subsets,tumor marker levels,and postoperative complications were compared between the two groups.Results After 3 weeks of treatment,forced expiratory volume in one second(FEV1)and forced vital capacity(FVC)were higher in the control group than before treatment(P<0.05).In the treatment group,FEV1,FVC,and FEV1/FVC were higher than before treatment(P<0.05),and all were higher than those in the control group at the same time point(P<0.05).After 3 weeks of treatment,the levels of carcinoembryonic antigen(CEA),cytokeratin 19 fragment antigen 21-1(CYFRA21-1),and carbohydrate antigen 125(CA125)were lower in both groups than before treatment(P<0.05),and all were lower in the treatment group than in the control group at the same time point(P<0.05).After 3 weeks of treatment,CD3+,CD4+,CD4+/CD8+,and CD8+improved in both groups compared with baseline(P<0.05).In the treatment group,CD3+,CD4+,and CD4+/CD8+were higher than in the control group at the same time point(P<0.05),whereas CD8+was lower than in the control group(P<0.05).Generalized estimating equations confirmed significant time effects and group intervention effects in T lymphocyte subset changes.After 3 weeks of treatment,CD3+,CD4+,and CD4+/CD8+were higher in the treatment group than in the control group,while CD8+was lower than in the control group(P<0.05).The total incidence of postoperative complications was 8.61%(5/58)in the treatment group and 20.69%(12/58)in the control group.The difference in the total incidence of complications between the two groups was not statistically significant(P>0.05).Conclusion Qingzao Jiufei Decoction can effectively improve pulmonary function,reduce serum tumor marker levels,and regulate immune function in patients after thoracoscopic sublobar resection for NSCLC,with favorable safety.关键词
