黄芪泽苓颗粒联合特利加压素治疗顽固性腹水疗效及对患者免疫相关因子的影响
Efficacy of Huangqi Zeling granules combined with terlipressin in the treatment of refractory ascites and its effects on immune-related factors

Abstract
Objective To observe the clinical efficacy of Huangqi Zeling granules combined with terlipressin in the treatment of refractory ascites and to evaluate its effects on immune-related factors and ascites depth.Methods A total of 96 patients with refractory ascites admitted to the Department of Gastroenterology of Chang'an Hospital from January 2021 to December 2023 were enrolled and randomly assigned to a treatment group(n=48,2 dropouts)and a control group(n=48,3 dropouts)using a random number table.The control group received terlipressin,while the treatment group received Huangqi Zeling granules in addition to terlipressin.Clinical efficacy,adverse reactions,ascites depth,abdominal circumference,24 h urine volume,immune-related factors(interleukin-8[IL-8],intercellular adhesion molecule-1[ICAM-1]),and liver function indicators(aspartate aminotransferase[AST],total bilirubin[TBil],and albumin[Alb])were recorded and compared between the two groups.Results The treatment group had a significantly higher total effective rate than the control group(89.13%[41/46]vs 68.89%[31/45],P<0.05).After 1 and 2 months of treatment,ascites depth,abdominal circumference,serum and ascitic fluid IL-8 and ICAM-1 levels,as well as serum AST and TBil levels,were lower than baseline in both groups(P<0.05),and were lower in the treatment group than in the control group at the same time points(P<0.05).After 1 and 2 months of treatment,24 h urine volume and serum Alb levels were higher than baseline in both groups(P<0.05),and were higher in the treatment group than in the control group at the same time points(P<0.05).The overall incidence of adverse reactions was 8.70%(4/46)in the treatment group and 13.33%(6/45)in the control group;there was no statistically significant difference between the two groups(P>0.05).Conclusion Huangqi Zeling granules combined with terlipressin is effective in treating refractory ascites.It can alleviate ascites-related symptoms,increase 24 h urine volume,promote recovery of liver function,and appears to be safe.Its mechanism may be related to the regulation of immune-related factors.关键词
