原发性PD患者心肌间碘苄胍摄取率与心率变异性的相关性
Correlation between myocardial metaiodobenzylguanidine uptake and heart rate variability in patients with Parkinson's disease

Abstract
Objective To investigate the correlation between myocardial metaiodobenzylguanidine(MIBG)uptake and heart rate variability(HRV)in patients with Parkinson's disease(PD),and analyze the relationships with disease duration,disease severity,and orthostatic hypotension.Methods A retrospective analysis was conducted on 56 patients diagnosed with primary PD at Beijing Friendship Hospital,Capital Medical University,from July 2021 to November 2025.Clinical data,Hoehn-Yahr(H-Y)stage,123I-MIBG myocardial scintigraphy results(early and delayed heart-to-mediastinum ratios(H/M)),and 24-hour ambulatory electrocardiogram HRV parameters,including time-domain indices(SDNN,SDNN-INDEX,rMSSD,PNN50)and frequency-domain indices(LF,HF,LF/HF),were collected.Spearman correlation analysis was used to evaluate the relationships among these parameters.Results No significant correlations were found between early(H/M15min)or delayed(H/M4h)MIBG uptake and disease duration or H-Y stage(P>0.05).Among HRV parameters,only SDNN showed a significant negative correlation with H-Y stage(r=-0.369,P=0.005),and the other parameters were not correlated with disease duration or H-Y stage(P>0.05).Both H/M15min and H/M4h were positively correlated with the LF/HF ratio(r=0.341,0.348,P=0.01,0.009,respectively).Orthostatic hypotension was present in 22(39.29%)patients.These patients exhibited significantly lower H/M15min,H/M4h,and LF values compared to those without orthostatic hypotension(P<0.05).Conclusion MIBG uptake in PD patients is positively correlated with the LF/HF ratio,suggesting an association between impaired cardiac sympathetic integrity and sympathovagal imbalance.Reduced delayed MIBG uptake and decreased LF values are closely related to the presence of orthostatic hypotension,offering potential clinical value for identifying patients at high risk for cardiovascular autonomic dysfunction.SDNN may serve as a potential indicator reflecting changes in autonomic function associated with overall PD progression.关键词
