基底动脉近端病变导致的轻型后循环梗死血管内治疗结局
Outcomes of endovascular treatment for mild posterior circulation stroke due to proximal basilar artery lesions

Abstract
Objective To investigate the safety and efficacy of endovascular treatment(EVT)in patients with minor posterior circulation infarction caused by proximal basilar artery lesions.Methods Patients with posterior circulation infarction due to proximal basilar artery lesions who underwent EVT admitted to the Stroke Center of Affiliated Hospital of Jiujiang University between January 2020 and December 2024 were retrospectively analyzed.The baseline clinical data,imaging findings(CTA,CTP,MRI,DSA),procedural details,and outcome measures were collected.Successful recanalization was defined as a modified thrombolysis in cerebral infarction(mTICI)grade of 2b-3,and good functional outcome was a defined as modified Rankin scale(mRS)score≤2 at 90 days.Results A total of 10 patients were included(6 male,mean age 65 years old).The median NIHSS score on admission was 4 points(range 1-6).The mean onset-to-puncture time was(14±3.7)hours.Aspiration thrombectomy was performed in 6 patients,stent retriever thrombectomy in 1 patient,and combined techniques in 3 patients.All patients underwent low-pressure balloon angioplasty after thrombectomy,and 2 patients received stent implantation.Successful recanalization was achieved in all patients(mTICI 3 in 8 patients and 2b in 2 patients).The median NIHSS score at discharge was 1.5 points,and the median NIHSS score at 90 days was 0 point.At 90-day follow-up,all patients had mRS score≤2,yielding the good functional outcome rate of 100%.Conclusion In carefully selected patients with minor posterior circulation infarction due to proximal basilar artery lesions,EVT demonstrates a high recanalization rate and favorable functional outcomes in this single center,small sample cohort,suggesting that this strategy is generally safe and feasible.关键词
