活血理气针刺法联合身痛逐瘀汤治疗气滞血瘀型腰椎间盘突出症的效果
Effects of acupuncture for promoting blood circulation and regulating qi combined with Shentong Zhuyu Decoction in the treatment of lumbar disc herniation of qi stagnation and blood stasis type

Abstract
Objective To investigate the efficacy of blood-activating and qi-regulating acupunc-ture combined with Shentong Zhuyu Decoction in the treatment of lumbar disc herniation(LDH)of qi stagnation and blood stasis type.Methods A total of 100 LDH patients admitted from January 2022 to March 2024 were divided into control group(conventional symptomatic supportive treatment com-bined with Shentong Zhuyu Decoction)and combination group(conventional symptomatic supportive treatment combined with Shentong Zhuyu Decoction and blood-activating and qi-regulating acupunc-ture)according to treatment regimens,with 50 cases in each group.Propensity score matching(PSM)method was used for 1∶1 matching of baseline data between the two groups,and finally 48 patients were included in each group.After 6 weeks of continuous treatment,clinical efficacy,traditional Chinese medicine syndrome scores,pain severity,lumbar function,safety,and disease recurrence were compared between the two groups.Results The total effective rate in the combination group was 93.75％(45/48),which was significantly higher than 79.17％(38/48)in the control group(P=0.037).After 6 weeks of treatment,the primary syndrome score and secondary syndrome score in the combination group were(13.35±1.63)and(5.14±0.58),respectively,which were significantly lower than(16.49±2.32)and(6.28±0.79)in the control group(P＜0.05).The Visual Analogue Scale(VAS)scores at 4 and 6 weeks in the combination group were(4.35±1.24)and(2.52±0.76),respectively,which were significantly lower than(4.89±1.35)and(3.42±1.14)in the control group(P＜0.05).After treatment,the Oswestry Disability Index(ODI)score in the combination group was(10.58±1.87),which was significantly lower than(14.47±2.76)in the control group,while the lumbar flexion-extension range and lumbar rotation range in the combination group were(75.65±14.43)° and(39.67±9.25)°,respectively,which were significantly higher than(68.83±12.28)° and(35.49±8.37)° in the control group(P＜0.05).The incidence of adverse reactions in the combination group was 16.67％(8/48),and that in the control group was 12.50％(6/48),with no statistically significant difference(P＞0.05).A 6-month follow-up of treatment-effective patients after treatment completion showed that the recur-rence rate in the treatment group was 8.89％(4/45),and that in the control group was 15.79％(6/38),with no statistically significant difference(P＞0.05).Conclusion Blood-activating and qi-regulating acupuncture combined with Shentong Zhuyu Decoction demonstrates favorable clinical efficacy in treating LDH of qi stagnation and blood stasis type,can effectively alleviate lumbocrural pain symptoms,improve lumbar mobility function,exhibiting a good safety profile.关键词
