心房颤动病人护理诊断的混合性研究
A mixed-methods study on nursing diagnoses for patients with atrial fibrillation

Abstract
Objective:To identify the primary nursing diagnoses for patients with atrial fibrillation.Methods:A mixed-methods study with a quantitative-priority design was employed.For the quantitative component,a convenience sample of 84 atrial fibrillation patients hospitalized in the cardiology department of a tertiary hospital in Hubei Province from July to December 2024 was enrolled.Data were collected via medical record review and standardized assessments.Frequency and percentage analyses were performed using SPSS 25.0.For the qualitative component,a purposive sample of 15 patients underwent semi-structured interviews.Nursing diagnoses were determined based on the NANDA International Nursing Diagnoses:Definitions and Classification,2021-2023.Results:A total of 714 nursing diagnoses were extracted from the quantitative study and classified into 15 categories,before catheter ablation,"risk of thrombosis(65.5％)"and"risk of metabolic syndrome(64.3％)"were the main ones;after the operation,"body movement disorder(100.0％)"and"risk of adult fall(88.1％)"were prominent.A qualitative study distilled 90 diagnostic expressions and consolidated them into 15 types of nursing diagnoses,supplementing unquantified questions such as"risk of reduced activity tolerance(86.7％)","risk of impaired cardiovascular function(80.0％)","anxiety(73.3％)",and"lack of knowledge(66.7％)".Integrated analysis showed that the combination of quantitative assessment and qualitative interviews could comprehensively identify perioperative nursing problems.Conclusions:Nursing diagnoses for atrial fibrillation patients exhibit stage-specific characteristics.The pre-operative phase focus on thrombosis and metabolic risks;the intra-operative phase requires management of pain and anxiety;and the post-operative phase centers on physical dysfunction and fall prevention.Psychosocial issues identified through qualitative research highlights the need for more individualized care planning.关键词
