彩色多普勒超声联合血清FGF-19、TRAb对分化型甲状腺癌的诊断价值
Diagnostic value of color Doppler ultrasound combined with serum FGF-19 and TRAb for differentiated thyroid cancer

Abstract
Objective To investigate the diagnostic value of color Doppler ultrasound combined with serum fibroblast growth factor 19(FGF-19)and thyrotropin receptor antibody(TRAb)for differentiated thyroid cancer(DTC).Methods Eighty patients with DTC admitted to this hospital from January 2023 to December 2024 were retrospectively selected as the DTC group,and 40 patients with benign thyroid nodules during the same period were included as the control group.Color Doppler ultrasound examination and measurement of se-rum FGF-19 and TRAb levels were performed in both groups.Kappa analysis was used to evaluate the consis-tency between diagnoses based on color Doppler ultrasound,serum FGF-19 and TRAb,alone and in combina-tion,and the pathological diagnosis.Receiver operating characteristic(ROC)curve analysis and fourfold table method analysis were employed to assess the diagnostic value of color Doppler ultrasound combined with ser-um FGF-19 and TRAb for DTC.Results Lesion boundary,calcification,internal structure,perilesional lymph node enlargement,echogenicity,morphology and maximum lesion diameter differed significantly between the two groups(P<0.05).Serum FGF-19 and TRAb levels were significantly higher in the DTC group than those in the control group(P<0.05).Significant differences in the proportions of high and low serum FGF-19 and TRAb expression were observed among patients with different TNM stages and lymph node metastasis status in the DTC group(P<0.05).The areas under the curves(AUCs)for serum FGF-19 and TRAb alone in diagnosing DTC were 0.835 and 0.838 respectively.The Kappa values for consistency between the patho-logical diagnosis and diagnoses based on color Doppler ultrasound,serum FGF-19,TRAb alone,and their com-bination were 0.646,0.460,0.534 and 0.870 respectively.The diagnostic accuracies of color Doppler ultra-sound,serum FGF-19,TRAb and their combination for DTC were 84.17%,75.83%,79.17%,and 94.17%re-spectively.The accuracy of combined detection in diagnosing DTC was higher than that of each indicator a-lone,and its sensitivity and specificity were higher than those of serum FGF-19 or TRAb alone(P<0.05).Conclusion Serum FGF-19 and TRAb levels are markedly elevated in patients with DTC.Color Doppler ul-trasound combined with serum FGF-19 and TRAb improves diagnostic performance for DTC and is helpful for early screening of DTC.关键词
