椎旁肌间隙入路钉棒内固定联合伤椎置钉治疗单节段胸腰椎骨折的疗效
The efficacy of Wiltse approach screw-rod internal fixation in the treatment of single-level thoracolumbar fractures

Abstract
Objective To investigate the efficacy of the pedicle screw-rod internal fixation combined with injured ver-tebrae screwing via interspinal muscle splitting approach(Wiltse approach)on the single-level thoracolumbar frac-tures.Methods The 35 patients with single-level thoracolumbar fractures were divided into the observation group(treated with Wiltse approach internal fixation combined with injured vertebrae screwing,20 cases)and the control group(treated with posterior median approach internal fixation combined with injured vertebrae screwing,15 cases).The surgical conditions,the ratio of the anterior vertebral height,Cobb angle of the injured vertebra,pain VAS,and ODI were compared between the two groups.Results All patients were followed up for 12-18 months.The opera-tion time,intraoperative bleeding volume,incision healing time,and postoperative ambulation time in the observation group were shorter(less)than those in the control group(P＜0.05).The anterior vertebral height ratio and Cobb angle of the injured vertebra:At 6 and 12 months after surgery,both groups improved,compared with those before sur-gery(P＜0.05),and there was no statistically significant difference between the two groups(P＞0.05).The pain VAS and ODI:At 6 and 12 months after surgery,two groups were lower than those before surgery(P＜0.05),and the data of observation group were lower than the control group(P＜0.05).Conclusions Compared with the poste-rior median approach,the Wiltse approach with internal fixation using screws and rods combined with injured verte-brae screwing for single-level thoracolumbar fractures has the advantages of less bleeding and shorter operation time,better maintenance of the stability of the thoracolumbar spine sequence,and improvement of the lumbar back activity function of patients.关键词
