两种穿刺方法在经皮椎体成形术治疗骨质疏松性椎体压缩骨折中的应用比较

Abstract
Objective To compare the therapeutic effects of unilateral two-plane puncture and bilateral two-plane puncture in percutaneous vertebroplasty(PVP)for osteoporotic vertebral compression fracture(OVCF).Methods The 84 patients with OVCFs who underwent PVP were divided into group A(The 48 cases were performed with bilat-eral two-plane puncture during the operation)and group B(The 36 cases were performed with unilateral two-plane puncture during the operation)based on the different puncture methods.The surgical conditions,bone cement leak-age ratio,pain VAS,ODI,and complications of the two groups were compared.Results All patients were followed up for 8-13(9.5±1.5)months.The operation time and the times of intraoperative fluoroscopy in group A were lon-ger(more)than those in group B(P＜0.01),while there were no statistically significant differences in the volume of bone cement injection and the rate of bone cement leakage between the two groups(P＞0.05).The bone cement dis-persion in group A was all excellent,while in group B,4 cases did not reach excellent grading due to poor dispersion of bone cement in the coronal plane,and the other cases were excellent.At 1 d after the operation and the last follow-up,the pain VAS and ODI of both groups improved,compared with those before the operation(P＜0.01),but there were no statistically significant differences between the two groups(P＞0.05).Conclusions PVP with unilateral two-plane puncture and bilateral two-plane puncture for treating OVCF can both achieve satisfactory therapeutic effects.The bilateral two-plane puncture method has a longer operation time and more fluoroscopic exposures,while the bone cement distribution is better;when the center-line of the coronal plane is far from the center in unilateral two-plane puncture,it is prone to poor distribution of bone cement in the coronal plane,and the bilateral two-plane puncture method is recommended for contralateral puncture.关键词
