ACBT联合阶梯式人文护理在口腔癌术后气道管理中的应用
Application of active cycle of breathing technique(ACBT)combined with stepwise humanistic care in postoper-ative airway management for oral cancer

Abstract
Objective:To evaluate the effect of active cycle of breathing techniques(ACBT)combined with stepwise humanistic care on airway management in patients with an artificial airway after radical surgery for oral cancer,and to provide evidence for establishing an oral-specific,physiology-psychology integrated airway management model.Methods:A total of 120 patients with an artificial airway after radical oral cancer surgery plus flap reconstruction,admitted between October 2024 and April 2025,were enrolled and divided into an observation group and a control group according to admission time,with 60 cases in each group.The control group received routine airway care based on a bundled protocol.The observation group re-ceived ACBT for airway clearance plus a four-step humanistic care model in addition to the routine care.The two groups were compared in terms of blood gas analysis,inflammatory markers,sputum clearance effectiveness,length of hospital stay,suc-cessful extubation rate,pulmonary infection rate,anxiety score,humanistic care satisfaction,and communication satisfaction.Results:On postoperative days 1,3,and 5,the observation group had significantly higher partial pressure of arterial oxygen(PaO2)than the control group,and on day 5 the observation group also had a significantly lower partial pressure of arterial carbon dioxide(PaCO2)(P<0.05).Inflammatory markers decreased more substantially in the observation group after surgery(P<0.05).The overall effective rate of sputum clearance was 98.3%in the observation group versus 83.3%in the control group(P<0.01).Hospital stay was shorter in the observation group(P<0.05).Anxiety scores,humanistic care satisfaction,and communication satisfaction were all better in the observation group(P<0.05).No adverse events such as flap injury or bleeding occurred in either group.Conclusion:Combining ACBT with stepwise humanistic care effectively improves oxygena-tion and ventilation function,reduces systemic inflammation,enhances airway clearance efficiency,shortens hospital stay,and alleviates anxiety in patients with an artificial airway after oral cancer surgery,even in the presence of aphasia.It also boosts treatment adherence and the overall patient experience.This model aligns well with the specialized nursing needs of post-op-erative oral cancer patients,is safe and effective.关键词
