摘要
Abstract
Objective To evaluate the therapeutic efficacy of tumor necrosis factor-α(TNF-α)antagonists in patients with ankylosing spondylitis(AS)-associated uveitis. Methods This was a multicenter retrospective case series study.A total of 87 patients with AS-associated uveitis who were treated in Baoji People's Hospital and Baoji Central Hospi-tal between December 2021 and December 2024 and had complete electronic medical records were included.Among them,75 were male and 12 were female, with a mean age of(30.7±13.2)years(range,16~52 years).Patients were divided into two groups according to whether they received TNF-α antagonist therapy. The control group included 43 patients who re-ceived conventional topical corticosteroid eye drops combined with nonsteroidal anti-inflammatory drugs(NSAIDs),while the study group included 44 patients who received TNF-α antagonists in addition to conventional treatment.Age,sex, and disease duration were collected.Aqueous humor cell count(AHCC),the best-corrected visual acuity(BCVA),Bath Ankylos-ing Spondylitis Disease Activity Index(BASDAI),and uveitis recurrence during the 12-month follow-up were compared be-tween the two groups before the treatment and at 1 week,3 weeks,2 months,3 months,6 months and 12 months after the treatment. Results There were no statistically significant differences in baseline characteristics between the two groups be-fore the treatment(all P>0.05).At 1 and 3 weeks after the treatment, there were no statistically significant differences in AHCC,logMAR BCVA,or BASDAI between the two groups(all P>0.05).At 2,3,6 and 12 months after the treatment,AHCC and BASDAI were lower in the study group than in the control group(all P<0.05).At 3,6 and 12 months after the treatment, logMAR BCVA was lower in the study group than in the control group(all P<0.05).During the 12-month follow-up,recurrence occurred in 20 patients in the control group, with a recurrence rate of 46.51%,and in 11 patients in the study group, with a recurrence rate of 25.00%.The difference between the two groups was statistically significant(P=0.036).Conclusions TNF-α antagonist therapy for AS-associated uveitis can reduce aqueous humor cell count, improve logMAR BCVA,lower the recurrence rate of uveitis, and reduce AS disease activity. It may represent an effective therapeu-tic strategy for patients with AS-associated uveitis.关键词
