Abstract
Objective To evaluate the short-term clinical effect of bifocal toric intraocular lenses implantation in cataract patients with high myopia and corneal astigmatism,and vector analysis of residual astigmatism and corneal astig-matism.Methods This was a retrospective case observation study.17 patients(26 eyes)with high myopia,cataracts,and regular corneal astigmatism who were treated at Northern Jiangsu People's Hospital between January 2023 and October 2023 were included.All eyes underwent phacoemulsification with toric bifocal intraocular lenses(IOL)implantation.Uncor-rected distance(5 m),intermediate(80 cm),and near(40 cm)visual acuity(logMAR),as well as best-corrected distance,in-termediate,and near visual acuity(logMAR)were recorded and compared preoperative and at 1 week,1 month,and 3 months postoperatively.Predicted versus actual postoperative residual total ocular astigmatism was analyzed.IOL axis rota-tion was evaluated at 1 and 3 months postoperatively.Statistical analysis was performed using repeated-measures one-way ANOVA or t-test in SPSS 26.0.Results Uncorrected distance,intermediate,and near visual acuity significantly improved at 1 week,1 month,and 3 months postoperatively compared to preoperative values(P＜0.001).Mean total ocular astigma-tism decreased from(1.24±1.09)D preoperative to(0.17±0.25)D at 3 months postoperatively(P＜0.001).The centroid value of total ocular astigmatism shifted from(0.26 D@123°±1.51 D)preoperative to(0.06 D@145°±0.30 D)postoperative.No significant difference was observed between predicted residual astigmatism(0.13±0.07 D;centroid:0.06 D@145°±0.30 D)and actual residual astigmatism at 3 months(0.17±0.25 D;centroid:0.04 D@71°±0.15 D;P=0.449).Preoperative corneal astigmatism(1.57±0.49 D;centroid:0.84 D@97°±1.45)D remained stable postoperatively(1.55±0.49 D;centroid:0.58 D@91°±1.55 D;P=0.636).IOL axis rotation was 2.0°(1.0°,3.0°)at 1 month and 2.5°(1.0°,4.0°)at 3 months(P=0.059).Con-clusions Toric bifocal IOLs provide satisfactory full-range visual acuity,effectively reduce postoperative residual astigma-tism,and demonstrate excellent rotational stability in highly myopic cataract patients with regular corneal astigmatism.关键词
