中青年心力衰竭患者性健康体验及沟通需求的质性研究
Experiences and communication needs of sexual health in young and middle-aged patients with heart failure:a qualitative study

Abstract
Objective To explore the experiences and communication needs of sexual health in young and middle-aged patients with heart failure,and to provide references for promoting sexual health communication and improving quality of sexual life in this group of population.Methods Using purposive sampling,a total of 33 participants,including young and middle-aged patients with heart fail-ure,their spouses,and medical staff from a tertiary grade-A hospital in Nanjing,were recruited for semi-structured interviews.Using Maslow's hierarchy of needs as a framework,the data were analyzed by directed content analysis.Results Five themes and thirteen sub-themes were extracted:coping with limited sexual function(disease symptom control,and adverse drug reaction management),nav-igating sexual safety concerns(safe behavior guidance,urgent events handling,and establishment of psychological safety),reconstruct-ing sexual intimacy amid relational imbalance(adaptation to role transition,and enhancement of intimate relationship),maintaining sex-ual dignity under challenges(self-recognition and acceptance,spouse understanding and respect,and social recognition and support),and breakthroughs under sexual autonomy suppression(overcoming communication barriers,ensuring autonomous decision-making,and safeguarding the value of life).Conclusion Young and middle-aged patients with heart failure experience multiple shocks to their sexual health,leading to complex and helpless inner experiences.Therefore,multidimensional communication support is urgently needed to alleviate their sexual health concerns.关键词
