Editorial Explanation for the Expert Consensus on Diagnosis and Treatment of Cerebral Small Vessel Disease with Integrated Chinese and Western Medicine

Abstract
In response to the fundamental and clinical issues such as diagnosis,etiology and pathogenesis in the process of traditional Chinese and Western medicine diagnosis and treatment of cerebral small vessel disease,the Hunan Provincial Academy of Chinese Medicine has taken the lead in collaborating with 27 research institutions,tertiary grade-A hospitals,and universities across the country,including Xiangya Hospital of Central South University and China Center for Science and Technology Development of Traditional Chinese Medicine.A multidisciplinary expert group was formed(consisting of 52 experts from neurology,imaging,rehabilitation,pharmacy,and methodology),and the compilation work was carried out strictly in grading of recommendations assessment,development and evaluation(GRADE)evidence grading system and the development specifications for expert consensus as well as the World Health Organization(WHO)guidelines.Through systematic literature retrieval,assessment of recommendation grading,GRADE evidence grading,and expert consultation(21 experts voted to determine 6 basic questions and 17 clinical questions),the disease definition,epidemiological characteristics,cranial MRI as the gold standard for diagnosis,clinical diagnosis,treatment methods,prognosis and care of cerebral small vessel disease were clarified and incorporated into the consensus formulation.The compilation process underwent multiple rounds of expert reviews both within and outside the group and one round of expert argumentation.Literature analysis and the Delphi method were adopted to ensure the rigor of the methodology and its clinical applicability.Through policy coordination,academic promotion,and dynamic revision mechanisms,the standardization of clinical application was promoted,providing a demonstration for the optimization of diagnosis and treatment of cerebral small vessel disease.关键词
