基于线粒体自噬探讨"阳化气,阴成形"理论指导下的帕金森病病机与治疗
Exploring the Pathogenesis and Treatment of Parkinson's Disease Based on the Theory of"Yang Qi Transformation and Yin Manifestation"from the Perspective of Mitochondrial Autophagy

Abstract
Parkinson's disease(PD)is a neurodegenerative disorder characterized by dopaminergic neuron degeneration in the substantia nigra and abnormal aggregation of α-synuclein(α-syn).Impaired mitophagy is a key pathological link in PD,contributing to disease progression by inducing disturbances in energy metabolism and the accumulation of pathological products such as α-syn and inflammasomes.From the perspective of the traditional Chinese medicine(TCM)theory"Yang Qi Transformation and Yin Manifestation",the function of mitophagy in maintaining energy metabolism and clearing damaged mitochondria represents the scientific connotation of"Yang Qi Transformation",namely its functions of warming and promoting actions.Meanwhile,the accumulation of pathological products caused by defective mitophagy reflects the pathological essence of"excessive Yin Manifestation"leading to phlegm and blood stasis stagnation.These two aspects form a vicious cycle of"functional abnormality—product accumulation"that exacerbates the disease.Based on this,therapeutic principles and methods of warming Yang and replenishing qi,along with removing blood stasis and resolving phlegm,can restore"Yang transforming qi",and dissipate the phlegm and stasis resulting from"excessive Yin constituting form,"thereby regulating Yin-Yang balance and exerting therapeutic effects on PD by modulating mitophagy.This article attempts to systematically elaborate the TCM understanding of PD pathogenesis from the theory of"Yang Qi Transformation".Focusing on the core imbalance of"insufficient Yang Qi Transformation"and"excessive Yin Manifestation",it explores the biological essence of this imbalance and further analyzes the intrinsic relationship between this theory and mitophagy dysfunction in PD.On this basis,it summarizes the intervention strategies of Chinese herbal medicines and formulas following the principles of warming Yang and replenishing qi,removing blood stasis and resolving phlegm,which regulate mitophagy,in order to provide new ideas for the treatment of PD.关键词
