Status quo and influencing factors of high-output stoma in patients with preventive ileostomy

Abstract
Objective:To investigate the status quo of high-output stoma(HOS)after preventive ileostomy in patients with middle and low rectal cancer and to analyze its influencing factors.Methods:Using the convenience sampling method,a total of 589 patients with middle and low rectal cancer who underwent preventive ileostomy in a tertiary Grade A specialized tumor hospital in Liaoning province from January to October 2025 were selected as the research subjects.The General Information Questionnaire,Nutrition Risk Screening 2002(NRS 2002),Self-Rating Anxiety Scale(SAS),Social Support Rating Scale(SSRS),and a Questionnaire on Early Postoperative Dietary Behavior were used to conduct the survey.Logistic regression analysis was performed to analyze the influencing factors of high-output stoma in patients with preventive ileostomy.Results:Ultimately,a total of 572 patients were included.Among them,135 cases experienced high-output stoma after preventive ileostomy,with an incidence rate of 23.60%.Logistic regression analysis results showed that residual small bowel length<150 cm(OR=3.516),preoperative nutritional risk(OR=2.851),early postoperative hypertonic liquid intake>500 mL/d(OR=2.428),early postoperative anxiety(OR=2.103),and low level of social support(OR=1.754)were the independent influencing factors for high-output stoma in patients with preventive ileostomy(P<0.05).Conclusions:The incidence of high-output stoma in patients with middle and low rectal cancer after preventive ileostomy is relatively high.Medical staff should pay close attention to patients with short residual small bowel length,nutritional risk and anxiety,mobilize social support systems,and strengthen the management and guidance of hypertonic liquid intake to reduce the risk of high-output stoma.关键词
