Abstract
Objective To investigate correlation between body mass index(BMI)and abnormal fasting glucose and lipid metabolism in elderly population, and to evaluate the predictive value of BMI for these metabolic abnormalities. Methods A total of 1035 elderly individuals(aged 64-92 years)who underwent physical examination at Danfeng Community Health Service Center in Nanning from January 2020 to January 2022 were retrospectively enrolled. They were divided into four groups according to BMI:underweight group, normal weight group, overweight group, and obese group.Demographic data, physical indicators, and metabolic comorbidities were collected.Fasting plasma glucose(FPG),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)were measured. In addition, intergroup comparison,Spearman correlation analysis, multivariate logistic regression analysis, and receiver operating characteristic(ROC)curve analysis were performed. Results There were statistically significant differ-ences in FPG,TG,LDL-C,and HDL-C levels,as well as in the prevalence of gender, waist circumference, hypertension, and fatty liver disease among different BMI groups(P<0.05).Correlation analysis showed that BMI was positively correlated with FPG(r=0.123)and TG(r=0.080),and negatively correlated with HDL-C(r=-0.207)(allP<0.01).Multivari-able logistic regression analysis showed that BMI was an independent factor associated with abnormal FPG(OR=1.540,95%CI:1.239-1.912),elevated TG(OR=1.201,95%CI:1.009-1.428),and reduced HDL-C(OR=1.633,95%CI:1.252-2.129)(all P<0.05).ROC analysis showed that the area under the curve(AUC)of BMI for predicting abnormal FPG,elevated TG,and reduced HDL-C was 0.612(95%CI:0.569-0.655),0.536(95%CI:0.495-0.577),and 0.624(95%CI:0.573-0.676),respectively.The AUC of BMI combined with FPG and HDL-C for predic-ting elevated TG was 0.709(95%CI:0.670-0.748),which was numerically higher than that of BMI alone. Conclusion Elevated BMI is independently associated with abnormal FPG,elevated TG,and decreased HDL-C in the elderly popula-tion,but not significantly associated with TC or LDL-C.BMI has certain screening value for glucose and abnormal lipid me-tabolism,but its predictive efficacy alone is limited. In clinical practice,it should be combined with waist circumference, blood glucose, and blood lipid indicators for comprehensive assessment.关键词
