Value of CEUS combined with serum CYFRA 21-1 in evaluating axillary lymph node metastasis in patients with invasive breast cancer

Abstract
Objective:To explore the value of contrast-enhanced ultrasound(CEUS)combined with serum cytokeratin 19 fragment(CYFRA 21-1)in evaluating axillary lymph node metastasis in patients with invasive breast cancer.Methods:A total of 133 patients with invasive breast cancer presenting with axillary lymph nodes enlargement who admitted to our hos-pital between June 2022 and May 2024 were enrolled retrospectively.According to the histopathological results of axillary lymph node metastasis,the patients were divided into a metastasis group(n=84)and a non-metastasis group(n=49).Clinical data of all patients were collected to analyze the independent influencing factors of axillary lymph node metastasis,and receiver operating characteristic(ROC)curves were used to analyze the diagnostic efficacy.Results:The tumor aspect ratio,maximum nodule diameter,and serum CYFRA 21-1 levels in the metastasis group were significantly higher than those in the non-metastasis group,whereas the baseline intensity(BI)and time-to-peak(TTP)on CEUS were significantly lower than those in the non-metastasis group(P＜0.05).Multivariable Logistic regression analysis demonstrated that the tumor aspect ratio,maximum nodule diameter,CEUS baseline intensity,TTP,and serum CYFRA 21-1 level were all independent predic-tors of axillary lymph node metastasis in patients with invasive breast cancer(P＜0.05).ROC curve analysis showed that BI,TTP,and CYFRA 21-1 level possessed distinct values in evaluating axillary lymph node metastasis,and the clinical-radiological combined prediction model achieved the highest overall diagnostic efficacy.Conclusion:Tumor aspect ratio,maximum nodule diameter,CEUS BI,TTP,and serum CYFRA 21-1 levels are closely related to the development of axil-lary lymph node metastasis.The combination of quantitative CEUS parameters and serum CYFRA 21-1 provides high diag-nostic value for axillary lymph node status in patients with invasive breast cancer.关键词
