玉龙县纳西族居民糖尿病疾病经济负担变化趋势研究
Trends in the economic burden of diabetes of the Naxi ethnic group residents in Yulong Naxi Autonomous County,Yunnan Province

Abstract
Objective To analyze the changing trends in the economic burden of diabetes among Naxi ethnic residents in Yulong Naxi Autonomous County,Yunnan Province.Methods A repeated cross-sectional de-sign was adopted,and Naxi permanent residents aged 35 years and above in Yulong Naxi Autonomous County,Yunnan Province were recruited by multistage stratified random sampling for a questionnaire survey and blood glu-cose testing.The incidence of catastrophic health expenditure and the incidence of poverty due to diabetes were used as indicators to measure the economic impact on households.Results The standardized prevalence rates of diabetes were 4.0％,5.6％,and 11.1％in 2013,2018,and 2023,respectively.The per capita direct,indi-rect,and economic burdens of diabetes were 6 444.5,714.0,and 7 158.5 yuan in 2013;9 764.8,662.4,and 10 427.2 yuan in 2018;and 9 646.0,286.0,and 9 932.0 yuan in 2023,respectively.The standardized preva-lence of diabetes showed an overall increasing trend from 2013 to 2023(x2=336.917,P＜0.001).An increas-ing trend was found in the per capita direct and economic burden of diabetes,whereas a decreasing trend was ob-served in the per capita indirect economic burden of diabetes.The incidence rates of household catastrophic health expenditure and poverty due to diabetes were 17.8％and 8.9％in 2013,15.0％and 7.1％in 2018,and 3.2％and 1.9％in 2023,respectively,both rates showing a downward trend(x2trend of household catastrophic health expenditure=25.715,P＜0.001;x2trend of poverty due to diabetes=10.558,P=0.001).Conclusions The impact of diabetes on the household economy was weakened over time among the Naxi ethnic minority in Yulong Naxi Autonomous County,while prevalence and per capita economic burden of diabetes increased over time.It is vital to enhance the preven-tion and management of diabetes,so as to reduce the economic burden of diabetes.关键词
