Abstract
Duplicated middle cerebral artery(DMCA)is a rare anatomical variant characterized by two independent middle cerebral artery(MCA)trunks arising from the terminal segment of the internal carotid artery.Acute occlusion associated with a DMCA is rare,and sequential occlusion of the two MCA trunks is even rarer.If patency of one trunk is interpreted as patency of the ipsilateral(MCA),occlusion of the other trunk may be missed,thereby delaying reperfusion therapy.This report described a patient with DMCA-related sequential occlusion of both MCA trunks.Because postoperative altered aortic arch anatomical changes after surgical repair of an aortic dissection made conventional peripheral arterial access difficult,percutaneous direct puncture of the left common carotid artery was initially planned.However,marked dilation of the left internal jugular vein limited the available puncture trajectory.Therefore,the left common carotid artery was exposed through a longitudinal cervical incision,punctured under direct vision,and cannulated with an arterial sheath for mechanical thrombectomy.Preoperative CT angiography showed patency of the left DMCA,whereas intraoperative DSA demonstrated occlusion of both the left MCA trunk and the DMCA.Both trunks were successfully recanalized after aspiration thrombectomy.This report summarized the diagnostic and therapeutic course of the patient and reviewed the relevant literature,aiming to provide a reference for imaging recognition and endovascular treatment strategies for DMCA-related occlusion.关键词
