Expert consensus on the diagnosis and treatment of advanced KRAS-mutant non-small cell lung cancer(2026 Edition)
摘要
Abstract
Kirsten rat sarcoma viral oncogene homolog(KRAS)is recognized as one of the key driver genes in non-small cell lung cancer,with its activating mutations being primarily observed at codons 12,13,and 61.Among these,KRASG12C is the most common subtype.KRAS mutations,especially the G12C mutation,can drive tumor cell proliferation,invasion,and metastasis.They are frequently associated with poor prognosis,primary resistance to traditional EGFR TKIs,and potential heterogeneity in the efficacy of immune checkpoint inhibitors(ICIs),with some subtypes characterized by a"cold tumor"immune microenvironment.In recent years,breakthroughs have been made in the development of covalent irreversible inhibitors targeting KRASG12C mutations.These drugs selectively and covalently bind and lock the KRASG12C protein,thereby providing targeted therapy options for patients with advanced NSCLC and leading to significantly improved objective response rates and progression-free survival.In order to standardize the clinical diagnosis and treatment pathway of KRAS-mutant NSCLC in China and promote the standardization of molecular testing and the rational application of innovative treatment strategies,Tumor marker professional committee of China anti-cancer association,Integrated oncology branch of the China anti-cancer association,Nanomedicine oncology professional committee of Shaanxi anti-cancer association integrated the latest evidence from evidence-based medicine domestically and internationally with local clinical practice experience and developed the Expert consensus on the diagnosis and treatment of advanced KRAS-mutantnon-small cell lung cancer(2026 Edition).The ultimate goal of the consensus is to provide guidance for the standardized and individualized diagnosis and treatment of KRAS-mutant NSCLC,thereby improving clinical benefit and long-term quality of life for patients.关键词
