肺癌术后患者创伤后成长轨迹及影响因素的纵向研究
A longitudinal study of the post-traumatic growth trajectory and influencing factors in lung cancer patients after surgery

Abstract
Objective To investigate the trajectories of post-traumatic growth(PTG)in patients after lung cancer surgery and to analyze the effects of factors such as temperament type on trajectory classes.Methods The study included patients who received initial treatment for lung cancer and underwent video-assisted thoracoscopic surgery at the Guangxi Medical University Cancer Hospital between February and November 2023.PTG levels were assessed on the day of discharge and at 1,3,6,and 9 months postoperatively.A latent class growth model(LCGM)and growth mixture model(GMM)were applied to identify distinct PTG trajectory classes,and multinomial Logistic regression was used to analyze the factors influencing different trajectory classes.Results The patients were categorized into the following four groups,which showed very different patterns of PTG change over 9 months after lung cancer surgery:stable high group(18.0%),high decreasing group(41.0%),low increasing group(16.7%),and stable low group(24.3%).Univariable analysis showed that disease stage,postoperative chemotherapy status,and temperament type were associated with trajectory classes.Multinomial Logistic regression analysis indicated that,with the stable high group as the reference group,receipt of postoperative chemotherapy and temperament type were factors influencing PTG trajectories.Conclusions PTG in patients after lung cancer surgery exhibits distinct trajectories.Chemotherapy status and temperament characteristics are associated with PTG trajectories.It is recommended that stratified intervention plans be developed based on chemotherapy status and temperament type to optimize rehabilitation management for patients after lung cancer surgery.关键词
