Progress in cellular immunotherapy for relapsed/refractory classical Hodgkin lymphoma

Abstract
Most patients with classical Hodgkin lymphoma(cHL)achieve durable remission after standard first-line therapy,but some patients develop primary refractory disease or relapse.Cellular immunotherapies based on chimeric antigen receptor T cells(CAR-T),Epstein-Barr virus-specific T cells(EBVST),and natural killer(NK)cells provide new therapeutic approaches for relapsed/refractory classical Hodgkin lymphoma(R/R cHL).Current evidence is derived mainly from phase Ⅰ and Ⅱ single-arm studies with small sample sizes.Clinical data are most extensive for CD30 CAR-T therapy,which can induce high response rates,although the durability of response after a single infusion remains limited.EBVST therapy has a favorable safety profile and is mainly applicable to patients with EBV-positive disease,but direct evidence in R/R Chl remains limited.AFM13-precomplexed NK cells have potential advantages including allogeneic sourcing,off-the-shelf availability,and relatively low acute immune toxicity,although long-term disease control remains limited.This review focuses on the biological basis of immune evasion and targets of cellular immunotherapy in R/R Chl,systematically evaluates the efficacy evidence for these three strategies,and summarizes optimization strategies and key aspects of safety management,with the aim of informing clinical treatment selection and future study design.关键词
