Relationship between quantitative analysis for MRI features, and intraoperative adhesion and pathological invasion in peritumoral edema of brain tissue of patients with meningioma
摘要
Abstract
Objective:To investigate the relationship between quantitative analysis for the features of magnetic resonance imaging(MRI),and intraoperative adhesion and pathological invasion of peritumoral edema of brain tissue of patients with meningioma. Methods:The clinical data of 180 patients who underwent surgery for meningioma at Huanggang Central Hospital were retrospectively enrolled from January 2023 to December 2025,and they were divided into a brain invasion group(36 cases)and a non-invasion group(144 cases)according to postoperatively pathological results. All patients underwent preoperatively multimodal MRI examination, and the imaging parameters of peritumoral edema were quantitatively analyzed.Univariate and multivariate logistic regression analyses were adopted to identify independent influencing factors that associated with brain invasion and severe adhesion. Results:MRI quantitative features indicated that the proportion of uneven enhancement(69.44%)and cystic necrosis(58.33%)of brain invasion group were significantly higher than those(38.19%and 22.92%)of non-invasion group, and the differences were statistically significant(x2=11.391,17.202,P<0.05).The male proportion, maximum diameter of edema, choline(Cho)value,Creatine(Cr)value, and fractional anisotropy(FA)values of peritumoral edema area of brain invasion group were significantly higher than those of non-invasion group, with statistically significant differences(t=6.846,9.570,7.633,6.178,4.321,P<0.05).However,the apparent diffusion coefficient(ADC),mean diffusivity(MD),axial diffusivity(AD),radial diffusivity(RD),and volume ratio of edema-to-tumor of brain invasion group were significantly lower than those of non-invasion group, and the differences were significant(t=3.384,3.405,2.452,3.245,11.510,P<0.05).Multivariate analysis revealed that uneven enhancement method, increased maximum diameter of edema, and elevated Cho/Cr ratio were independent risk factors for brain invasion and severe adhesion(OR=3.337,3.357,2.549,P<0.05),while regular morphology of tumor, higher ADC value, and larger volume ratio of edema-to-tumor were independent protective factors for them(OR=0.305,0.372,0.147,P<0.05).Conclusion:Quantitative MRI features of peritumoral edema of meningioma significantly relate to the degree of intraoperative adhesion of brain tissue, and the pathological invasion.Enhancement mode, edema range,Cho/Cr ratio, tumor morphology,ADC value, and volume ratio of edema-to-tumor can be used as important imaging indicators for predicting tumor invasiveness and adhesion severity before surgery, which can provide a basis for optimizing surgical strategy.关键词
