Policy Progress and Innovative Model of Pharmaceutical Care for Chronic Disease Management by Licensed Pharmacists in Community Pharmacies

Abstract
Objective To provide reference for licensed pharmacists in China to carry out specialized pharmaceutical care for chronic disease management in the setting of community pharmacies.Methods Relevant domestic and international policies and literature were systematically searched.The current status of pharmaceutical care for chronic disease management provided by licensed pharmacists in community pharmacies both domestically and internationally was reviewed.The core challenges,policy progress and practice models of pharmaceutical care in China were analyzed,and recommendations for role transformation and service quality improvement of licensed pharmacists in community pharmacies were proposed.Results At present,pharmaceutical care for chronic disease management in community pharmacies in China faces multiple challenges,including unclear definition of rights and responsibilities,lack of multidisciplinary collaboration,delayed linkage of patient diagnosis and treatment information,insufficient professional competence of licensed pharmacists,and absence of reimbursement mechanisms.Meanwhile,a series of domestic policies have provided important strategic opportunities for community pharmacies to undertake the drug supply and pharmaceutical care for chronic disease patients from medical institutions.The service models of chronic disease medication therapy management(MTM)in community pharmacies mainly include the primary service model(medication consultation and health guidance),the mainstream service model(MTM services),and innovative service models combining hospital-pharmacy collaboration with digital empowerment.Conclusion It is recommended to promote institutional development and clarify the responsibilities of licensed pharmacists in pharmaceutical care for chronic disease management;to improve the quality of pharmaceutical care and collaboratively develop standards for MTM by licensed pharmacists;and to enhance the initiative of pharmacists by implementing incentive policies for pharmaceutical care for chronic disease management in community pharmacies.In this way,the resource advantages of community pharmacies can be fully utilized to alleviate the pressure on medical institutions.关键词
