基于静息态fMRI对膝骨关节炎慢性疼痛患者局部神经活动及全脑功能连接特征的探究
Exploration of local neural activity and whole-brain functional connectivity in patients with knee osteoarthritis-related chronic pain based on resting-state fMRI

Abstract
Objective:To investigate the alterations in resting-state local neural activity and whole-brain functional connectivity(FC)in patients with knee osteoarthritis(KOA)-related chronic pain and to examine their associations with clinical symptoms.Methods:Thirty-one patients with KOA(KOA group)and 36 healthy controls(HC group)were included.Fractional amplitude of low-frequency fluctuation(fALFF),regional homogeneity(ReHo)values were compared.Brain regions showing significant group differences were selected as ROIs for FC analysis,and correlations between imaging metrics and clinical scale scores were examined.Results:Compared with the HC group,the KOA group showed significantly increased fALFF values in the bilateral postcentral gyrus and left precuneus(voxel-level P<0.001,cluster-level FDR-corrected P<0.05),and significantly increased ReHo value in the left postcentral gyrus(voxel-level P<0.001,cluster-level FWE-corrected P<0.05).When the differential cluster in the right postcentral gyrus identified by the fALFF analysis was used as ROI1,FC between ROI1 and the left inferior parietal lobule was significantly enhanced in the KOA group(voxel-level P<0.001,cluster-level FWE-corrected P<0.05).And when the differential cluster in the left postcentral gyrus identified by the ReHo analysis was used as ROI2,FC between ROI2 and the right postcentral gyrus was significantly enhanced in the KOA group(voxel-level P<0.001,cluster-level FWE-corrected P<0.05).The scores of each clinical scale were not significantly correlated with the fLAFF,ReHo values,and FC of the differential brain regions(all P>0.05).Conclusion:KOA-related chronic pain may be associated with the abnormality of the somatosensory network centered on the postcentral gyrus,which involves the alterations in bilateral sensory-motor coupling and parietal sensory integration.关键词
