MRI IVIM对妊娠中晚期脑室扩张胎儿脑组织扩散与灌注功能的研究
Analysis on diffusion and perfusion function of brain tissue in fetuses with ventriculomegaly in middle and late stages of pregnancy by IVIM

Abstract
Objective:To analyze the changes in the brain water molecular diffusion and perfusion in fetuses with ventriculomegaly in middle and late stages of pregnancy using MRI intravoxel incoherent motion(IVIM).Methods:Forty-three fetuses with slight ventriculomegaly(SVM group)and 9 fetuses with moderate ventriculomegaly(MVM group)were enrolled,and 44 normal fetuses were recruited as the control group.All fetuses in the three groups underwent IVIM examination.The pure diffusion coefficient(D),pseudo-diffusion coefficient(D*),and perfusion fraction(f)in different brain regions were compared among 3 groups.ROC curves were drawn to evaluate the efficacy of the indicators with group differences in the differential diagnosis of ventricular dilatation and its degree.Results:Compared with the control group,significantly decreased D values were observed in the frontal white matter,parietal white matter and temporal white matter in both the SVM group and MVM group(all P<0.05).The D value in the frontal white matter had significant difference between the MVM group and the SVM group(P<0.05).ROC curve analysis demonstrated that D values of the frontal,parietal and temporal white matter could distinguish the control group from the SVM group,with the frontal white matter showing the highest performance(AUC=0.89,P<0.05).Conclusion:The degree of reduction in the D value of the frontal white matter can serve as an indicator for evaluating white matter injury in fetuses with ventriculomegaly,which facilitates clinical monitoring of the progression of fetal cerebral white matter injury.关键词
