针刺治疗常年性变应性鼻炎的脑效应机制fMRI初探
A preliminary fMRI study on brain effect mechanisms of acupuncture in treating perennial allergic rhinitis

Abstract
Objective:To explore the brain effect mechanisms of acupuncture in treating perennial allergic rhinitis(PAR)based on fMRI regional homogeneity(ReHo)and functional connectivity(FC)analyses.Methods:Nineteen PAR patients(PAR group)and 19 healthy subjects(healthy group)were enrolled.The PAR group received standardized acupuncture treatment three times per week for 4 weeks on Yingxiang(LI20),Yintang(GV24+),Fengchi(GB20),Dazhui(GV14),Feishu(BL13),Pishu(BL20),Shenshu(BL23),Hegu(LI4),Zusanli(ST36).The rhinoconjunctivitis quality of life questionnaire(RQLQ),total nasal symptom score(TNSS),and total non-nasal symptom score(TNNSS)were used to assess clinical symptom before and after treatment.Resting-state fMRI data were acquired for ReHo and FC analyses.ReHo value was compared between the PAR group and the healthy group at baseline,as well as FC value within the PAR group before and after treatment.The correlation between FC difference values in differential brain regions and clinical symptom improvement was analyzed.Results:The total effective rate of the PAR group was 68.40%(13/19).The RQLQ score,TNSS and TNNSS in the PAR group were significantly lower after treatment and at 30-day follow-up compared to before treatment(all P<0.05).The ReHo value in the right superior frontal gyrus was significantly higher in the PAR group than that in the healthy group before treatment.After treatment,FC between the right superior frontal gyrus and the right angular gyrus,supramarginal gyrus,superior temporal gyrus increased to varying degrees compared with baseline.Further analysis revealed that the RQLQ score(practical problems domain)difference value in the PAR group showed a positive correlation with the FC difference values before and after treatment(ρ=0.476,P=0.039).Conclusions:Acupuncture is effective for the treatment of PAR.PAR patients may exhibit enhanced FC in the posterior part of the right superior frontal gyrus,and acupuncture treatment alters the synergistic function between this region and multiple key nodes across the brain.关键词
