杨天鹏手法联合圆针刀治疗膝骨关节炎的疗效观察
Observation of the Therapeutic Effect of Yang Tianpeng's Manipulation Combined with Circular Needle Knife on Knee Osteoarthritis Based on the Theory of Equal Emphasis on Muscles and Bones

Abstract
Objective:This study explores the clinical efficacy of Yang Tianpeng's manipulation combined with circular nee-dle knife based on the theory of equal emphasis on muscles and bones for the treatment of knee osteoarthritis(KOA).Methods:A total of 98 patients with KOA admitted from November 2023 to May 2025 were randomly divided into an cir-cular needle knife group and a combination therapy group using a random number table.The circular needle knife group adopts independently developed circular needle knife treatment;the combination therapy group was treated with circular needle knife combined with Yang Tianpeng's manipulation.Both groups received treatment for 4 consecutive weeks.Com-pare the visual analogue scale(VAS)scores,traditional Chinese medicine syndrome scores,Western Ontario and McMaster University osteoarthritis index(WOMAC)scores,AIMS2-SF scores,inflammatory factors(C-reactive protein(CRP),interleukin-6(IL-6)),and clinical efficacy before and after intervention,as well as monitor the occur-rence of adverse reactions between two groups.Results:In terms of pain,after 1,2,and 4 weeks of treatment,the VAS scores of the combination therapy group were lower than those of the circular needle knife group,and the differences were statistically significant(P＜0.05).In terms of traditional Chinese medicine symptom scores,the combination therapy group had lower scores in pain,stiffness,swelling,restricted mobility,and lumbar and knee soreness and weakness than the circular needle knife group(P＜0.05).In terms of joint function,quality of life,and inflammatory factors,the combination therapy group had lower average levels of WOMAC scores,CRP,and IL-6 compared to the circular needle knife group(P＜0.05),while there was no statistically significant difference between the AIMS2-SF groups(P＞0.05).In terms of clinical efficacy,the combination therapy group was higher than the circular needle knife group(Z=2.920,P＜0.05).However,there was no statistically significant differ-ence in the total effective rate between the two groups(x2=3.161,P＞0.05).There was no statistically significant differ-ence in the incidence of adverse reactions between the two groups(x2=0.005,P＞0.05).Conclusion:Yang Tianpeng's integrated approach-combining circular needle knife therapy with syndrome differentiation for early-to mid-stage KOA patients presenting liver-kidney deficiency syndrome-more effectively alleviate joint pain,improves joint function and range of motion,and reduce inflammatory factor levels.The short-term efficacy is superior to that of circular needle knife therapy alone,while its long-term outcomes require extended follow-up validation.关键词
