Effects of Antiretroviral Therapy on Changes in Body Weight and Body Fat in People Living with Human Immunodeficiency Virus

Abstract
The widespread use of antiretroviral therapy(ART)has transformed human immunodeficiency virus(HIV)infection from a fatal infectious disease into a chronic condition requiring long-term standardized treatment and continuous management.However,excessive or rapid weight gain observed in people living with HIV(PLWH)during treatment is closely associated with an increased risk of metabolic abnormalities such as insulin resistance and type 2 diabetes mellitus,as well as cardiovascular disease.This review summarizes the epidemiological characteristics of ART-associated weight gain,the metabolic mechanisms underlying weight gain and body fat distribution changes following ART initiation,the influence of different drug classes,and the patterns and management strategies of changes in body weight and body fat.It emphasizes that visceral adipose tissue accumulation in PLWH is a stronger predictor of cardiovascular and metabolic risks than overall body weight.Given the current absence of clear guideline recommendations for weight management during ART,this review summarizes potential intervention strategies,including early monitoring,regimen adjustment,lifestyle modification,and the use of metabolic agents.With the increasing burden of metabolic comorbidities in PLWH,management of body weight,fat distribution and metabolic risk should become an integral part of long-term comprehensive care for PLWH.关键词
