依西美坦联合卵巢功能抑制治疗在高复发风险绝经前乳腺癌中的应用价值
Clinical Value of Exemestane Combined with Ovarian Function Suppression in Premenopausal Breast Cancer at High Risk of Recurrence

Abstract
Objective:To evaluate the efficacy and safety of exemestane combined with ovarian function suppression(OFS)in premenopausal patients with breast cancer at high risk of recurrence and to assess its effects on ovarian function,tumor marker levels,and inflammatory factor levels.Methods:We retrospectively included 90 premenopausal patients with breast cancer at high risk of recurrence who were treated at our hospital between January 2023 and June 2024.According to the treatment regimen,patients were assigned to the control group(n=44;OFS alone)or the observation group(n=46;exemestane combined with OFS).Both groups received 6 consecutive courses of treatment.Ovarian function indicators[estradiol(E2)and follicle-stimulating hormone(FSH)],tumor markers[carcinoembryonic antigen(CEA)and β2-microglobulin(β2-MG)],and inflammatory factors[interleukin-2(IL-2),tumor necrosis factor-α(TNF-α),and interleukin-8(IL-8)]were compared between the two groups before and after treatment.Patients were followed,and progression free survival(PFS),the 1-year cumulative survival rate,quality of life[assessed using the Karnofsky Performance Status scale(KPS)],and the incidence of adverse reactions were recorded.Results:After treatment,FSH levels and KPS scores were higher in the observation group than in the control group(P<0.05),whereas E2,CEA,β2-MG,IL-2,TNF-α,and IL-8 levels were lower(P<0.05).Mean PFS was longer and the 1-year cumulative survival rate was higher in the observation group than in the control group(P<0.05).No significant between-group differences were observed in the incidences of rash,gastrointestinal reactions,arthralgia,or hepatic and renal function impairment(P>0.05).Conclusion:Exemestane combined with OFS effectively reduces estrogen levels,prolongs PFS,and improves quality of life in premenopausal patients with breast cancer at high risk of recurrence.关键词
