摘要
Abstract
Objective:To investigate an appropriate bevacizumab dosing regimen and its safety in the treatment of recurrent ovarian cancer, thereby providing evidence to guide clinical use. Methods:Clinical data from 150 patients with recurrent ovarian cancer treated in the Department of Gynecology at our hospital between 2022 and 2023 were retrospectively analyzed.According to the bevacizumab dose, patients were divided into the low-dose group(5 mg/kg,q2w),medium-dose group(10 mg/kg,q2w),and high-dose group(15 mg/kg,q3w),with 50 patients in each group. The first efficacy assessment was performed after 6 weeks of treatment.Efficacy was subsequently reassessed every 6 weeks until disease progression or intolerable toxicity.Logistic regression analysis was used to identify independent factors influencing treatment efficacy. The objective remission rate(ORR),disease control rate(DCR),progression free survival(PFS),and incidence of adverse reactions were compared among the three groups. Results:The ORR and DCR were 64.0%and 88.0%in the medium-dose group and 66.0%and 90.0%in the high-dose group, respectively, and were higher than the corresponding rates in the low-dose group(42.0%and 68.0%;P<0.05).No significant differences in ORR or DCR were observed between the medium-and high-dose groups(P>0.05).Median PFS was longer in the medium-dose group(8.5 months)and high-dose group(8.8 months)than in the low-dose group(6.2 months;P<0.05),with no significant difference between the medium-and high-dose groups(P>0.05).The incidences of hypertension and proteinuria in the high-dose group(36.0%and 28.0%,respectively)were higher than those in the low-dose group(14.0%and 8.0%)and medium-dose group(18.0%and 12.0%;P<0.05).No significant differences in the incidences of gastrointestinal reactions, bleeding,or fatigue were observed among the three groups(P>0.05).Multivariable Logistic regression analysis, with the low-dose group as the reference, showed that the medium-dose group(OR=2.310,95%CI:1.240~4.320,P=0.009)and high-dose group(OR=2.450,95%CI:1.320~4.560,P=0.004)were independently associated with a higher likelihood of treatment effectiveness(CR+PR).Conclusion:The 10 mg/kg,q2w bevacizumab regimen provided efficacy comparable to the 15 mg/kg,q3w regimen and superior to the 5 mg/kg,q2w regimen, while being associated with lower incidences of hypertension and proteinuria than the high-dose regimen. In this study, the comprehensive efficacy and safety of this regimen were relatively good, which can provide reference for clinical dose selection, but further prospective research is still needed for verification.关键词
