Association between serum alpha-Klotho levels and bone mineral density in middle-aged adults based on NHANES database

Abstract
Objective To investigate the association between serum α-Klotho levels and bone mineral density(BMD)in middle-aged adults,and provide epidemiological evidence for the prevention and management of osteoporosis.Methods A cross-sectional study was conducted based on the National Health and Nutrition Examination Survey(NHANES)database from 2011-2016.The study population consisted of middle-aged adults aged 40-59 years.Bone mineral density was measured at six sites using dual-energy X-ray absorptiometry(DXA):total body,pelvis,thoracic spine,lumbar spine,trunk and left leg.Serum α-Klotho levels served as the exposure variable,while whole-body and regional BMD served as outcome variables.Multivariable linear regression and piecewise linear regression models were em-ployed to analyze the associations,with adjustment for potential confounders including demographic characteristics,bio-chemical indicators and lifestyle factors.Stratified analyses were performed to explore association across various popula-tions.Results After adjusting for potential confounders,serum α-Klotho levels showed significant positive associations with thoracic spine BMD(β=0.047,95%CI:0.021-0.073)and trunk BMD(β=0.036,95%CI:0.012-0.059).These associations were more pronounced in males aged 40-49 years,non-Hispanic white individuals,those with normal body weight and those without a history of diabetes or hypertension.Piecewise linear regression analysis revealed a nonlinear association between α-Klotho and BMD:when α-Klotho levels were below the threshold,a positive association with thoracic spine BMD was observed;when α-Klotho levels exceeded the threshold,positive associations with thoracic spine,lumbar spine,pelvis and trunk BMD were noted.Conclusion Serum α-Klotho level is positively associated with thoracic spine and trunk BMD,with stronger associations observed in males aged 40-49 years,non-Hispanic white individuals,those with normal body weight and those without a history of diabetes or hypertension.These findings suggest that α-Klotho may serve as a predictive indicator for BMD assessment in specific populations.关键词
