老年脆性骨折合并衰弱患者跌倒事件的时间与空间分布特征及影响因素
Temporal and spatial distribution characteristics and influencing factors of fall events in older adults with fragility fractures and frailty
摘要
Abstract
Objective To investigate the current status of falls and analyze the influencing factors among elderly patients with fragility fractures and frailty.Methods According to the inclusion and exclusion criteria,245 elderly pa-tients with fragility fractures and frailty were recruited from the orthopedic ward of Zibo Central Hospital between January and May 2025.Data were collected using a general information questionnaire,the Fall Risk Assessment Scale for the Eld-erly,the Mini Nutritional Assessment-Short Form(MNA-SF),the Basic Activities of Daily Living(BADL)scale and the Simple Five-Item Scoring Scale.Results The average fall risk score of the patients was(7.63±4.85)points,with 198 cases(80.8%)classified as moderate risk.Age(＞80 years),sarcopenia,nutritional status and self-care ability were identified as the main influencing factors(P＜0.05).Sarcopenia was positively correlated with fall risk(r=0.734,P＜0.05),while nutritional status was negatively correlated with fall risk(r=-0.610,P＜0.05).Regarding the temporal and spatial distribution of falls,the afternoon(12∶00-18∶00)was the peak period,accounting for 37.55%;51.42%of falls occurred outdoors,and the leading causes of falls were unsteady gait(28.57%),slipping(26.53%)and leg weakness(14.28%).Conclusion The fall risk in elderly patients with fragility fractures complicat-ed by frailty is at a moderate level.It is recommended to establish a multidisciplinary collaborative fall prevention early warning management system in clinical practice with emphasis on early identification of elderly patients dependent in self-care or with sarcopenia,in order to achieve stratified and precise intervention.关键词
