Study on the Therapeutic Efficacy of Comprehensive Rehabilitation Treatment in Young Infants with Global Developmental Delay and Hypertonia

Abstract
Objective To investigate the differences in the therapeutic effects of comprehensive rehabilitation among children with global developmental delay(GDD)and hypertonia at different months of age,and to provide evidence-based basis for early intervention strategies.Methods A retrospective cohort study was conducted.A total of 142 children first diagnosed with GDD and hospitalized in the Department of Neurorehabilitation of Hebei Children's Hospital from January 2022 to May 2024 were enrolled.According to the age at first diagnosis,they were divided into the 3-6 months group(n=88),the 7-12 months group(n=31)and the 13-18 months group(n=23).All children received 6 months of comprehensive rehabilitation treatment.The spasticity of the triceps surae muscle was assessed using the modified Ashworth scale(MAS),and the developmental quotients(DQ)of gross motor and fine motor domains were evaluated with the Gesell developmental schedule.The therapeutic effect was determined by comparing the above indicators before and after treatment.Results After treatment,the MAS scores in all three groups were lower than those before treatment(P<0.05).The MAS score in the 3-6 months group was lower than those in the 7-12 months group and the 13-18 months group,and the MAS score in the 7-12 months group was lower than that in the 13-18 months group(P<0.05).After treatment,there was a statistically significant difference in the normalization rate of muscle tone among the three groups(79.54％vs 54.83％vs 13.04％,P<0.05).The differences in DQ values of gross motor and fine motor domains before and after treatment in the 3-6 months group were higher than those in the 7-12 months group and the 13-18 months group(P<0.05).Conclusion The rehabilitation efficacy of children with GDD and hypertonia is closely related to the timing of intervention,and the age of 3-6 months is the golden therapeutic window.Early comprehensive rehabilitation can significantly reduce muscle tone,increase the normalization rate of muscle tone and improve motor function.关键词
