Effects of Posterior Decompression and Interbody Fusion with Pedicle Screw-Rod Fixation on Spinal Alignment Correction and Neurological Function Recovery in Elderly Patients with Degenerative Scoliosis

Abstract
Objective To analyze the effects of posterior decompression and interbody fusion with pedicle screw-rod fixation at different segments on spinal alignment correction and neurological function recovery in elderly patients with degenerative scoliosis.Methods This retrospective cohort study enrolled 196 elderly patients with degenerative scoliosis who underwent posterior decompression and interbody fusion with pedicle screw-rod fixation at the Department of Orthopedics,the 904th Hospital of the PLA Joint Logistics Support Force from May 2022 to January 2025.Patients were divided into a short-segment group(≤3 fixed segments,n=98)and a long-segment group(>3 fixed segments,n=98)according to the number of fixed segments,and each group received corresponding segmental posterior decompression and interbody fusion with pedicle screw-rod fixation.Perioperative parameters,spinal alignment correction parameters,and neurological function recovery were observed and recorded in both groups before and after surgery.Results The operation time and hospital stay of the short-segment group were shorter than those of the long-segment group,and the intraoperative blood loss,number of fused segments,and postoperative drainage volume were lower in the short-segment group than in the long-segment group(all P<0.05).At 12 months postoperatively,the Cobb angle,sagittal imbalance,Oswestry disability index and visual analogue scale score in both groups were lower than preoperative values(P<0.05),while the lumbar lordosis angle and Japanese Orthopaedic Association score were higher than preoperative values(P<0.05).The incidence of postoperative complications in the short-segment group was lower than that in the long-segment group(5.10％vs 14.29％,χ2=4.721,P=0.030).Conclusion Posterior decompression and interbody fusion with pedicle screw-rod fixation is an effective surgical procedure for elderly patients with degenerative scoliosis.Short-segment and long-segment fixation yield comparable clinical outcomes,but the former is associated with less surgical trauma and fewer complications.For patients with mild to moderate scoliosis and a Cobb angle of less than 40°,short-segment fixation should be prioritized to avoid excessive surgery.关键词
