乳酸与白蛋白比值联合APACHE Ⅱ评分在脓毒症患者预后评估中的价值
The value of lactate to albumin ratio combined with APACHE Ⅱ score in the prognosis assessment of sepsis patients

Abstract
Objective To explore the prognostic value of the lactate to albumin ratio(LAR)combined with acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score in patients with sepsis.Methods A total of 76 patients with sepsis who were admitted to Wuxi Hospital of Traditional Chinese Medicine from July 2022 to July 2025 were selected.They were divided into death group(n=30)and survival group(n=46)based on their 28-day survival outcomes.The correlation between each risk factor and the 28-day mortality of patients was evaluated through Spearman correlation analysis.Using multivariate Logistic regression analysis,the influencing factors of 28-day mortality in patients with sepsis were investigated.The receiver operating characteristic(ROC)curve was drawn to evaluate the predictive value of each indicator for prognosis.Results Spearman correlation analysis indicated that age,coronary heart disease,liver damage,sequential organ failure assessment score,APACHE Ⅱ score,LAR were positively correlated with the 28-day mortality of sepsis patients(P<0.05),while albumin was negatively correlated with 28-day mortality(P<0.001).The results of the multivariate Logistic regression analysis showed that APACHE Ⅱ score and LAR were independent risk factors for 28-day mortality in patients with sepsis(P<0.05).The ROC curve results showed that area under the curve(AUC)for predicting 28-day mortality in sepsis patients by LAR and APACHE Ⅱ scores were 0.932 and 0.737,respectively.The AUC of the combined prediction of two methods was 0.946,with a sensitivity of 93.3%and a specificity of 82.6%.Conclusion The APACHE Ⅱ score and LAR are both independent risk factors for 28-day mortality in patients with sepsis,and their combined application can more effectively assess the prognosis of patients.关键词
