黄芪下瘀血汤加味治疗气虚血瘀型代谢相关脂肪性肝病患者的临床研究
Retrospective clinical study of modified Huangqi Xiayuxue decoction in the treatment of metabolic dysfunction-associated fatty liver disease with Qi-deficiency and blood-stasis syndrome

Abstract
Objective:To comprehensively evaluate the clinical efficacy of modified Huangqi xiayuxue Decoction(HD)in treating Metabolic dysfunction-associated fatty liver disease(MAFLD)with qi-deficiency and blood-stasis syndrome and its impact on Metabolic Syndrome(MetS).Methods:A retrospective study was conducted involving 127 MAFLD patients with qi-deficiency and blood-stasis syndrome.According to the actual treatment regimen,patients were assigned to a treatment group(n=65)and a control group(n=62).The control group received health education and conventional Western liver-protecting therapy,while the treatment group received additional HD based on the control group's treatment.The treatment course exceeded 24 weeks for both groups.Changes in liver function[alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyl transferase(GGT)],blood lipids[triglycerides(TG),total cholesterol(TCh),low-density lipoprotein cholesterol(LDL-C)],liver ultrasound B,controlled attenuation parameter(CAP),liver stiffness measurement(LSM),fibrosis-4 index(FIB-4),body mass index(BMI),MetS components,and traditional Chinese medicine(TCM)syndrome scores were compared before and after treatment.Results:After treatment,the improvement in liver function(ALT,AST,GGT),blood lipids(TG,TCh,LDL-C),BMI,CAP,and LSM in the treatment group was significantly superior to that in the control group(all P＜0.05).The total effective rates for improvement based on liver ultrasound B and CAP in the treatment group were 64.6％and 67.7％,respectively,both significantly higher than those in the control group(37.1％and 40.3％)(both P＜0.05).Regarding MetS,the improvement in the number of metabolic abnormality components,fasting blood glucose,and systolic blood pressure in the treatment group was also significantly better than that in the control group(P＜0.05).Disease spectrum analysis showed that 55.0％of patients with significant fibrosis at baseline in the treatment group achieved LSM reversal,which was significantly higher than the 15.8％in the control group(P=0.011).The total effective rate for TCM syndrome efficacy in the treatment group(86.2％)was significantly higher than that in the control group(19.4％)(P＜0.001).No adverse events were reported in either group during the treatment period.Conclusion:HD can significantly improve liver function,blood lipids,hepatic fat deposition,and fibrosis degree,effectively reduce BMI,regulate systemic metabolic status,and significantly alleviate TCM clinical symptoms in MAFLD patients with qi-deficiency and blood-stasis syndrome,demonstrating good safety.关键词
