Exploring the traditional Chinese medicine treatment of primary immune thrombocytopenia based on the"deficiency fire and latent fire"theory in Lixu Yuanjian

Abstract
Primary immune thrombocytopenia(ITP)is a common autoimmune hemorrhagic disorder in clinical practice.In traditional Chinese medicine(TCM),it is generally classified under categories such as Zidian(紫癜)and Xuezheng(血证).Clinically,treatment often focuses on hemostasis and the elimination of blood stasis,which can alleviate bleeding symptoms to a certain extent.However,recurrent and protracted disease courses as well as unstable therapeutic efficacy remain major challenges,significantly affecting patients'quality of life.Given its characteristics of Vital Qi(气)deficiency and chronic damage involving the collaterals,ITP can also be attributed to the category of Xulao(虚劳).Nevertheless,studies that systematically interpret and treat ITP based on Xulao theory remain relatively limited.In recent years,TCM approaches have explored ITP from perspectives such as Zang-Fu(脏腑)deficiency,Qi-blood imbalance,and Yin-Yang(阴阳)disharmony.Although some progress has been made,the overall theoretical framework still requires further integration and refinement.Lixu Yuanjian(《理虚元鉴》),a classical work on Xulao,is particularly distinguished by its fire-based theoretical system.Among its contents,the chapter"discussion on deficiency fire and latent fire"provides especially insightful interpretations of deficiency-related pathogenesis,offering valuable perspectives for understanding the mixed deficiency-excess nature and recurrent features of ITP.Based on the pathophysiological characteristics of deficiency-excess complexity in ITP,and integrating Western medical insights regarding immune dysfunction,decreased platelet production,and increased destruction,this study systematically analyzes the application of the"deficiency fire and latent fire"theory from three aspects:theoretical origin,pathogenesis,and therapeutic principles.Accordingly,the theory is summarized as"deficiency fire should be supplemented,and latent fire should be cleared".A therapeutic strategy is proposed that emphasizes warming and tonifying the kidney,strengthening the spleen to reinforce Vital Qi,combined with clearing lung fire and nourishing Yin to stop bleeding.This approach achieves a dynamic balance of supplementation within clearing and clearing within supplementation.The proposed framework not only helps explain the clinical features of recurrence,deficiency-excess complexity,and the difficulty in balancing tonification and reduction in ITP,but also provides a novel theoretical basis and clinical perspective for optimizing TCM treatment strategies for this condition.关键词
