Pofessor ZHANG Jinfeng's experience in treating endometrial polyps-induced infertility from the perspective of"deficient Qi stagnation"

Abstract
This article summarizes the clinical experience of Professor ZHANG Jinfeng,a renowned traditional Chinese medicine(TCM)doctor,in treating infertility caused by endometrial polyps(EP)from the perspective of"deficient Qi(气)stagnation".EP is a common gynecological disease in women of reproductive age,presenting with symptoms such as menorrhagia,prolonged menstrual duration,irregular vaginal bleeding,intermenstrual bleeding,and infertility,which significantly impact women's reproductive health and quality of life.Epidemiological data indicate that the current prevalence of EP among Chinese women is approximately 24.0%to 25.0%.Among the infertile population,the detection rate of EP is particularly prominent.For women with unexplained infertility,the probability of being diagnosed with EP through hysteroscopy is 16.5%to 26.5%.The detection rates for primary infertility and secondary infertility patients are 3.8%to 38.5%and 1.8%to 17.0%respectively,suggesting a close association between EP and infertility.Professor ZHANG Jinfeng starts from the"deficient Qi stagnation"theory and believes that the essence of the pathogenesis of EP-related infertility is kidney deficiency and blood stasis,with kidney deficiency being the root cause and blood stasis as the manifestation.The kidney stores essence and governs reproduction.If the kidney Qi is deficient,the Chong(冲)and Ren(任)meridians lose nourishment resulting in weakened blood circulation and the formation of"deficient Qi".When deficient Qi impairs the ability to carry blood,over time it leads to blood stasis.The obstruction of the uterine cavity by blood stasis causes polyps,which is called"stagnation".The obstruction of the uterine cavity by blood stasis prevents the implantation of embryo,thus causing infertility.The clinical approach is based on the fundamental principles of nourishing the kidneys and invigorating the blood,and advocating for phased and cyclical treatment.During the treatment of polyps,Professor ZHANG uses self-formulated prescriptions aimed at resovling blood stasis and eliminating polyps;in the fertility stage,she recommends treatment based on different phases of the menstrual cycle,dividing the cycle into four phases:during the menstrual period,treat with Wuwei Tiaojing San(五味调经散)to activate blood,resolve stasis,and regulate menstruation;during the late menstruation,treat with Zhuyun Yihao Fang(助孕1号方)to tonify the Kidney,replenish essence,and nourish blood;during the intermenstrual period,treat with Xuefu Zhuyu Wan(血府逐瘀丸)to regulate Qi,activate blood,and promote ovulation;during the premenstrual period,treat with Zuo Tai Fang(坐胎方)to tonify the kidney,nourish blood,and support embryo implantation.Professor ZHANG applied the above-mentioned treatment methods and achieved ideal clinical efficacy.A medical case is attached herein for the benefit of colleagues.关键词
