基于数据挖掘分析小儿推拿治疗积滞的选穴、手法应用规律
Analysis of manipulation patterns and acupoint selection rules in pediatric tuina for food stagnation via data mining

Abstract
Objective:To explore the manipulation patterns and acupoint selection rules in pediatric tuina for food stagnation via data mining.Methods:A computerized search was conducted in China National Knowledge Infrastructure(CNKI),SinoMed,Wanfang,VIP Database,PubMed,Cochrane Library,Web of Science,and Embase for clinical research literature on pediatric tuina for food stagnation(up to May 2025).After screening,a database of pediatric tuina prescription was established using Excel 2019 for frequency analysis of acupoints and manipulations.SPSS Modeler 18.0 and SPSS 26.0 were employed for association rule analysis,cluster analysis,and factor analysis,and results were visualized.Results:A total of 118 articles were included,involving 84 acupoints and 22 manipulations.Acupoints such as the spleen meridian,spine,abdomen,Banmen(板门),and four transverse creases were frequently used,while tuina manipulations such as pushing,kneading,rubbing,and pinching were commonly applied.The selected acupoints were mainly distributed among specific pediatric tuina points,the Ren meridian(任脉),foot Taiyang(太阳)bladder meridian,and foot Yangming(阳明)stomach meridian,with the majority concentrated in the upper extremities,dorsolumbar region,and thoracoabdomen area.Association rule analysis showed that the acupoints"spleen meridian-Banmen""spleen meridian-four transverse creases"and"spleen meridian-inner eight trigrams"had strong associations;the tuina manipulations"pushing-kneading","pushing-massaging",and"pushing-pinching"were found to have strong associations.Cluster analysis revealed 6 effective clusters for acupoints and 4 for manipulations.Factor analysis revealed that the acupoints and tuina manipulations yielded 8 and 4 valid common factors respectively.Conclusion:In pediatric tuina for the treatment of food stagnation,the spleen meridian,Banmen,and other core acupoints,along with pushing-based combined manipulations and other core methods,adhere to the treatment principle of"strengthening the spleen to aid digestion,and eliminating food stagnation to promote Qi(气)flow".The synergistic effects of tuina manipulations and meridian acupoints enhance the efficacy of strengthening the spleen and eliminating food stagnation,providing a reference for clinical research.关键词
