Research progress on TCM in regulating mitochondrial autophagy for heart failure based on the theory of"Yin Ping Yang Mi"

Abstract
Heart failure(HF)is the terminal stage of various cardiovascular diseases,and its occurrence and progression which are closely related to energy metabolism disorders,oxidative stress,and myocardial remodeling.However,the overall pathological regulatory mechanisms remain to be fully elucidated.The traditional Chinese medicine(TCM)theory of"Yin Ping Yang Mi(阴平阳秘)"emphasizes the dynamic coordination of Yin(阴)and Yang(阳)to maintain physiological homeostasis,which aligns closely with the concepts of energy homeostasis and cellular quality control recognized in modern medicine.Recent studies have found that mitophagy,as a crucial regulatory pathway for maintaining cardiomyocyte energy metabolism and mitochondrial quality,plays a key role in the occurrence and progression of HF.Based on the"Yin Ping Yang Mi"theory,this article systematically reviews the molecular regulatory mechanisms of mitophagy and its pathological significance in HF imbalance,clarifying its intrinsic correspondence with the pathogenesis concept of concept of"Yang Wei Yin Xian(阳微阴弦)".Furthermore,starting from TCM traditional Chinese medicine treatment approaches such as reinforcing Yang and benefiting Qi(气),suppressing Yin to eliminate turbidity,and combining reinforcement with inhibition,the article summarizes experimental and mechanistic evidence showing how Chinese medicine regulates mitophagy through multiple pathways and targets,improves energy metabolism,inhibits oxidative stress,and suppresses myocardial remodeling.This article suggests that mitophagy homeostasis is an important cellular-level manifestation of"Yin Ping Yang Mi"and its imbalance is one of the key mechanisms driving the progression of HF.This theoretical integration provides a new perspective and theoretical basis for understanding HF pathogenesis through the combination of traditional Chinese and Western medicine and for precise intervention using TCM.关键词
