右旋糖酐铁联合重组人促红素改善白血病患者化疗后贫血的临床研究
Clinical trial of iron dextran combined with recombinant human erythropoietin in improving anemia after chemotherapy in leukemia patients

Abstract
Objective To observe the clinical efficacy and safety of iron dextran injection combined with recombinant human erythropoietin injection in improving anemia in leukemia patients after chemotherapy.Methods The patients with anemia after chemotherapy for leukemia admitted to our hospital were selected as the research subjects.The patients were divided into the control group and the treatment group according to the random number table method.The control group received ferrous succinate tablets(oral administration,0.1 g each time,twice a day)combined with recombinant human erythropoietin injection(subcutaneous injection,150 IU·kg-1,three times a week)for treatment,while the treatment group received iron dextran injection(intravenous drip,determined the total amount according to the dosage table)combined with recombinant human erythropoietin injection(the same as the control group)for treatment.The treatment course for both groups was 4 weeks.The clinical efficacy,anemia-related indicators,iron metabolism indicators,inflammatory indicators and transfusion status of the two groups were compared,and the safety was evaluated.Results A total of 120 patients were included,with 60 cases in each group.The total effective rates of treatment in the treatment group and the control group were 88.33％(53 cases/60 cases)and 73.33％(44 cases/60 cases),respectively.There was a statistically significant difference between the two groups(P＜0.05).After treatment,the hemoglobin(Hb)levels of the treatment group and the control group were(112.56±9.08)and(104.83±10.12)g·L-1,respectively;the mean red cell volume(MCV)was(84.60±5.22)and(81.89±5.94)fL,respectively;the mean red cell hemoglobin concentration(MCHC)was(321.76±14.49)and(315.78±16.31)g·L-1,respectively;the serum ferritin(SF)was(121.59±15.63)and(38.71±7.14)μg·L-1,respectively;the transferrin saturation(TSAT)was(37.24±5.78)and(29.55±4.60)％,respectively;the levels of interleukin-6(IL-6)were(26.33±3.22)and(28.20±5.48)pg·mL-1,respectively;the tumor necrosis factor(TNF-α)was(35.86±4.42)and(38.14±6.58)pg·mL-1,respectively.Comparisons of these indicators between the two groups all showed statistically significant differences(P＜0.05,P＜0.01,P＜0.001).The blood transfusion rates of the treatment group and the control group were 1.67％(1 cases/60 cases)and 5.00％(3 cases/60 cases),respectively;the total incidence of adverse drug reactions was 5.00％(3 cases/60 cases)and 11.67％(7 cases/60 cases),respectively;there was no statistically significant difference between the two groups in the above two indicators(both P＞0.05).Conclusion Intravenous injection of iron dextran solution and recombinant human erythropoietin solution for treating anemia in leukemia patients after chemotherapy showed better results in terms of improving the overall clinical effectiveness rate,alleviating anemia,iron metabolism and inflammation compared to the oral administration of ferrous succinate tablets combined with recombinant human erythropoietin regimen.Moreover,there was no significant difference in the incidence of adverse reactions between the two groups,and their safety profiles were similar.关键词
