FCGR2B、FCGR3A和FCGR3B基因多态性与弥漫大B细胞淋巴瘤患者利妥昔单抗血药浓度及疗效的相关性研究
Study on the correlation between the FCGR2B、FCGR3A and FCGR3B gene polymorphisms and rituximab plasma concentration and efficacy in patients with diffuse large B-cell lymphoma

Abstract
Objective To explore the effect of immunoglobulin G Fc receptors(FcγRs)encoding genes(FCGR2B,FCGR3A,FCGR3B)polymorphisms on the concentration and treatment efficacy of rituximab injection.Methods Patients with newly diagnosed diffuse large B-cell lymphoma receiving rituximab,cyclophosphamide,doxorubicin,vincristine and prednisone(R-CHOP)regimen were enrolled.Rituximab concentration was detected by enzyme linked immunosorbent assay.Haploview 4.2 was used to select tag single nucleotide polymorphisms(tagSNPs),and genotypes was performed using matrix assisted laser desorption/ionization time of flight mass spectrometry.Intergroup comparison and univariate analysis were employed to assess the impact of FcγRs coding gene polymorphisms on rituximab concentration.Univariate logistic analysis,Kaplan Meier survival curves and Log rank test were used to evaluate the association between FcγRs coding gene polymorphisms and short/long term efficacy.Results A total of 97 patients was enrolled.For FCGR3A rs10917571,the CC group included 37 patients and AA+AC group included 58 patients,2 patients' genotype not available(NA).The 5 year overall survival(OS)rate of CC group and AA+AC group were 75.68％(28 cases/37 cases)and 94.83％(55 cases/58 cases),respectively.Patients with the CC genotype had significantly shorter OS[HR=4.98,95％confidence interval(CI):1.35-18.42,P＜0.05].None of the six tagSNPs showed a significant association with rituximab trough concentration(Ctrough)(all P＞0.05).Conclusion The FCGR3A rs10917571 CC genotype is associated with shorter overall survival(OS)of rituximab injection therapy,suggesting its potential utility as a reference marker for individualized treatment.关键词
