摘要
Abstract
Objective To observe the impact and safety of the combination of butorphanol tartrate injection and esoxicodone hydrochloride injection on the incidence of respiratory depression during painless gastroscopy. Methods Patients scheduled for painless gastroscopy were randomly divided into the control group and the treatment group. The treatment group was first intravenously injected with 0.2 mg·kg-1 of esilectaminone hydrochloride injection and 10 μg·kg-1 of tartaric butorphanol injection, and then 1.5 mg·kg-1 of propofol emulsion injection was intravenously injected 5 minutes later;the control group was slowly intravenously injected with the same dose of normal saline and 10 μg·kg-1 of tartaric butorphanol injection, and then 1.5 mg·kg-1 of propofol emulsion injection was intravenously injected 5 minutes later. The two groups were compared in terms of anesthetic efficacy, anesthesia conditions, recovery conditions,peri-anesthetic circulatory indicators, stress response-related indicators, analgesic effect, safety evaluation, and anesthesia satisfaction. Results A total of 92 patients were enrolled in the treatment group, with 4 dropouts, and finally 88 patients were included in the statistical analysis;90 patients were enrolled in the control group, with 2 dropouts, and finally 88 patients were included. The anesthesia induction time in the treatment group and the control group was(1.28±0.29)and(1.41±0.32)min,respectively;the dosage of propofol was(173.67±32.28)mg and(186.21±40.36)mg,respectively;the heart rate(HR)at the time of loss of consciousness(T1)was(78.15±7.45)and(75.22±7.64)beats·min-1,respectively;the mean arterial pressure(MAP)at T1 was(88.18±7.37)mmHg and(85.67±7.26)mmHg,respectively;the blood oxygen saturation(SpO2)at T1 was(98.54±2.15)%and(97.71±2.02)%,respectively;the VAS scores at awakening for the treatment group and the control group were(1.15±0.28)and(1.38±0.35),respectively;the VAS scores 1 hour after awakening were(1.67±0.36)and(1.81±0.42)scores,respectively;the VAS scores 2 hours after awakening were(1.43±0.42)and(1.64±0.45)scores,respectively;the VAS scores 4 hours after awakening were(0.89±0.25)and(1.02±0.30)scores,respectively.Compared treatment group with control group, the differences of above indicators were all statistically significant(P<0.05,P<0.01,P<0.001).The adverse drug reactions in the treatment group and the control group included respiratory depression, hypotension, choking, body movement, tachycardia, bradycardia, nausea and vomiting;the incidence rates of adverse drug reaction were 13.64%(12 cases/88 cases)and 52.27%(46 cases/88 cases),with statistically significant difference(P<0.001).Conclusion Butorphanol tartrate injection combined with esketamine hydrochloride injection for painless gastroscopy has a rapid onset, causes minimal respiratory and circulatory depression, effectively inhibits the patient's stress response, and shows high safety.关键词
