Influence of bone condition at implant sites on implant accuracy by dynamic navigation technology

目的:探讨种植位点骨条件对动态导航技术下种植精度的影响.方法:回顾分析 2022~2023 年动态导航种植手术的下颌后牙区牙列缺失患者,纳入 26 例患者(30 颗种植体).测量种植位点骨密度、骨嵴顶皮质骨厚度、骨嵴顶下 3、6、9 mm颊舌皮质骨和松质骨的厚度及皮质骨与松质骨厚度比值,并与种植偏差值进行Pearson相关分析及多重线性回归分析.结果:骨密度与植入角度偏差呈负相关(r=-0.471,P<0.01),且有负因果关系(b=-0.005,P<0.01),植入角度偏差与骨嵴顶皮质骨厚度呈负相关(r=-0.400,P<0.05),与骨嵴顶下 3 mm颊舌皮质骨厚度呈负相关(r=-0.372,P<0.05),与骨嵴顶下3 mm皮质骨厚度/松质骨厚度比例呈负相关(r=-0.362,P<0.05).结论:种植术区颌骨密度越高,皮质骨厚度越高,动态导航技术下种植体植入角度偏差越小.

Objective:To evaluate the influence of bone condition at implant sites on the accuracy of implant place-ment with dynamic navigation technology.Methods:A retrospective analysis was conducted on patients with dental arch de-fects in the posterior mandibular region who underwent dynamic navigation implant surgery from 2022 to 2023.A total of 30 implant from 26 patients were studied.The bone density at the implant site,as well as crestal cortical bone thickness,bucco-lingual cortical and cancellous bone thickness at 3,6,and 9 mm below the crestal bone were measured.The ratio of cortical bone thickness to cancellous bone thickness at different depths was calculated.Pearson correlation analysis and multiple line-ar regression analysis were performed to assess the correlation between these bone conditions and implant deviation data.Re-sults:The bone density was significantly negatively correlated with the implant angle deviation(r=-0.471,P<0.01),indica-ting a significant causal relationship(b=-0.005,P<0.01).The implant angle deviation was also negatively correlated with crestal cortical bone thickness(r=-0.400,P<0.05),as well as with buccolingual cortical bone thickness at 3 mm below the crestal bone(r=-0.372,P<0.05),and the ratio of cortical bone thickness to cancellous bone thickness at 3 mm below the crestal bone(r=-0.362,P<0.05).Conclusion:Higher bone density and greater cortical bone thickness in the implant area are associated with smaller deviations in implantation angles by dynamic navigation technology.
