作业治疗对提高脑瘫患儿接受普通教育的适应性观察OA

目的:观察作业治疗对学龄前期脑瘫儿童接受普通教育应用适应性的影响.方法:将60例脑瘫患儿随机分为对照组(n=30)和观察组(n=30),对照组接受普通学前教育(幼儿园),观察组在此基础上给予作业治疗,分别于治疗前、治疗3个月后、治疗6个月后采用精细运动功能评定量表(FMFM)、婴儿-初中生社会适应能力量表(S-M)从上肢精细运动能力值、独立生活、运动能力、作业操作能力、交往能力、参加集体活动、自我管理7个方面评价脑瘫儿童的学龄前期教育适应程度,同时评价作业治疗对提高学龄前期脑瘫儿童接受普通教育适应行为的可行性.结果:两组治疗后3、6个月FMFM总分及各分区评分显著提高(P<0.05),且观察组改善优于对照组(P<0.05);两组治疗后3、6个月S-M总分、独立生活、作业操作能力、参加集体活动、自我管理均显著提高(P<0.05),且观察组优于对照组(P<0.05);但在运动能力、交往能力两组治疗前与治疗后3个月、6个月分值对比有统计学意义(P<0.05),组间差异无统计学意义(P>0.05).结论:学龄前期脑瘫儿童早期接受普通教育有利于改善其适应行为,结合作业治疗可效果更加显著,值得临床推广应用.

Objective:To observe the effect of occupational therapy on adaptability of preschool children with cerebral palsy receiving general educa-tion.Methods:60 children with cerebral palsy were randomly divided into control group(n=30)and observation group(n=30),the control group re-ceived general pre-school education(Kindergarten),and the observation group received occupational therapy on the basis of the control group.Be-fore treatment,3 months and 6 months after treatment,the fine motor function assessment scale(FMFM)and the social adaptability scale for infants and junior middle school students(S-M)were used to evaluate the preschool education adaptation of children with cerebral palsy from seven aspects:fine motor ability of upper limbs,independent living,motor ability,operation ability,communication ability,participating in collective activities and self-management.At the same time,the feasibility of occupational therapy on improving the adaptive behavior of preschool children with cerebral palsy was evaluated.Results:After 3 and 6 months of treatment,the total score and every subarea of FMFM in the two groups were significantly in-creased(P<0.05),and the improvement in observation group was superior to that in control group(P<0.05).The total score of S-M,independent liv-ing,operation ability,participation in group activities and self-management were significantly increased in the two groups at 3 and 6 months after treatment(P<0.05),and there were significant differences in the scores of motor ability and communication ability between the two groups before treatment and 3 and 6 months after treatment(P<0.05),but there were no significant differences in the scores of motor ability and communication ability between the two groups(P>0.05).Conclusion:Early general education is beneficial to the improvement of adaptive behavior of preschool chil-dren with cerebral palsy,and occupational therapy is more effective,which is worthy of clinical application.
