基于锥形束CT的下牙槽神经前袢测量分析OACSTPCD
Measurement and analysis of the anterior loop of the inferior alveolar nerve based on cone beam CT
目的 应用锥形束 CT(cone beam CT,CBCT)对下牙槽神经前袢(anterior loop of the inferior alveolar nerve,AL)进行测量分析,以降低颏孔区种植手术风险.方法 选取2015年11月至2016年4月于贵州医科大学附属口腔医院口腔修复种植科就诊的拟行种植手术的415例患者临床和影像学资料进行回顾性分析.依据性别、年龄(20~<45岁组、45~<60岁组、60~75岁组)和下颌前磨牙缺失情况(下颌前磨牙完整组、下颌前磨牙缺失组)进行分组,采用CBCT斜横截面重建法分析不同组间AL发生率及长度差异.结果 415例患者中有338例患者存在AL,AL 发生率为 81.4%;AL 长度为 0.25~5.40 mm,平均(2.07±0.95)mm;左侧下颌骨 AL 长度[(2.24±1.10)mm]大于右侧[(1.98±1.08)mm],差异具有统计学意义(t=3.040,P=0.002).下颌前磨牙完整组AL发生率(83.8%)明显高于下颌前磨牙缺失组(59.0%),且20~<45岁组、45~<60岁组、60~75岁组AL发生率依次为86.3%、78.6%、52.5%,差异具有统计学意义(P<0.05).但不同性别患者的AL发生率和AL长度,以及不同年龄、不同下颌前磨牙缺失情况患者的AL长度比较,差异均无统计学意义(均P>0.05).结论 AL发生率较高、长度变异大,下颌前磨牙完整患者和年轻患者在颏孔区进行种植手术时风险可能更高,术前应用CBCT检测是否存在AL及其长度对降低颏孔区种植手术潜在并发症的发生具有重要意义.
Objective To reduce the risk of implantation in the mental foramen by measuring and analyzing the anterior loop of the inferior alveolar nerve(AL)based on cone beam CT(CBCT).Methods Totally 415 patients with clinical and imaging data were selected and retrospectively analyzed,who were scheduled for implant surgery in the Department of Prosthodontics and Implantology,Affiliated Stomatological Hospital of Guizhou Medical University from November 2015 to April 2016.The AL was measured and analyzed by CBCT.They were classified according to gender,age(20-<45 years old group,45-<60 years old group,60-75 years old group)and mandibular premolars dentition status(mandibu-lar premolars dentate group,mandibular premolars edentulous group),the differences in prevalence of anterior loop and the length of the anterior loop for each patient were analyzed among different groups.Results AL was present in 338 of 415 patients,with an prevalence rate of 81.4%.The AL length was 0.25-5.40 mm,with a mean length of(2.07±0.95)mm.AL length on the left mandible[(2.24±1.10)mm]was greater than that on the right[(1.98±1.08)mm],and the difference was statistically significant(t=3.040,P=0.002).The prevalence rate of AL in mandibu-lar premolars dentate group(83.8%)was significant-ly higher than that in mandibular premolars edentu-lous group(59.0%).The incidence of AL in 20-<45 years old group,45-<60 years old group and 60-75 years old group was 86.3%,78.6%and 52.5%,respectively,and the differences were statistically significant(P<0.05).However,there were no statistically sig-nificant differences in the prevalence and length of AL between patients with different gender,or in AL length among dif-ferent age groups and between mandibular premolars dentate group and mandibular premolars edentulous group(P>0.05).Conclusion The prevalence rate of AL is high and the average range of AL length exhibits a wide variation.Pa-tients with mandibular premolars dentate and younger patients may have higer risk for clinical implant in the mental fora-men.Using CBCT scans to detect the AL and its length is of great significance in reducing the potential complications of implant surgery in the mental foramen.
