目的:探究控制论为导向的教育干预对行膝关节置换术的膝骨性关节炎(KOA)患者情绪状态和生活质量的影响.方法:选取2020年10月 2021年10月某院收治的86例行膝关节置换术的KOA患者,随机分为对照组和观察组,各43例,对照组给予常规干预,观察组在对照组基础上给予控制论为导向的教育干预.比较两组情绪状态[焦虑自评量表(SAS)和抑郁自量表(SDS)]、心理弹性[心理弹性量表(CD-RISC)]、自我效能[膝关节自我效能量表(K-SES)]、病情控制情况[西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分和美国特种外科医院膝关节(HSS)评分]及生活质量[健康状况调查简表(SF-36)].结果:干预后,相较于对照组,观察组SAS评分和SDS评分明显较低(t=-5.102,-4.035;P<0.001),CD-RISC量表各维度评分均明显较高(t=4.024,7.182,7.096;P<0.001),K-SES 量表各维度评分均明显较高(t=5.524,4.676,5.222,6.752;P<0.001),WOMAC 评分明显较低(t=-4.467,P<0.001),HSS评分明显较高(t=6.834,P<0.001),SF-36各维度评分均明显较高(t=4.978,9.641,7.020,6.090,6.532,8.609,5.387,6.544;P<0.001).结论:针对行膝关节置换术的 KOA 给予控制论为导向的教育干预,可改善患者情绪状态,增强心理弹性,提升自我效能,促进病情恢复,提高生活质量.

Objective:To investigate the effect of cybernetic-oriented educational intervention on the emotional state and quality of life of patients with knee osteoarthritis(KOA)who underwent knee arthroplasty.Methods:Eighty-six KO A patients with knee arthroplasty admitted to our hospital from October 2020 to October 2021 were select-ed and randomly divided into a control group and an observation group,43 cases each,with the control group given conventional intervention and the observation group given cybernetic-oriented educational intervention on the basis of the control group.The two groups were compared for emotional status[Self-Assessment Scale for Anxiety(SAS)and Depression Self-Scale(SDS)],psychological resilience[Psychological Resilience Scale(CD-RISC)],self-effi-cacy[Knee Self-Efficacy Scale(K-SES)],disease control[Western Ontario and McMaster University Osteoarthri-tis Index(WOMAC)score and Hospital for Special Surgery(HSS)Knee of America score]and quality of life[Health Status Survey Short Form(SF-36)].Results:After the intervention,compared to the control group,the observation group had significantly lower SAS scores and SDS scores(t=-5.102,-4.035;P<0.001),significantly higher scores on all dimensions of the CD-RISC scale(t=4.024,7.182,7.096;P<0.001),and significantly higher scores on all di-mensions of the K-SES scale(t=5.524,4.676,5.222,6.752;P<0.001),significantly lower of theWOMAC scores(t=-4.467,P<0.001),significantly higher of the HSS scores(t=6.834,P<0.001),and significantly higher of the SF-36 scores on all dimensions(t=4.978,9.641,7.020 6.090,6.532,8.609,5.387,6.544;P<0.001).Conclusion:Cy-bernetic-oriented educational interventions for KO A with knee arthroplasty can improve patients'emotional state,enhance psychological resilience,improve self-efficacy,promote recovery,and improve quality of life.
