目的:应用多导睡眠图监测(polysomnography,PSG)结果客观分析急性缺血性脑卒中(Acute ischemic stroke,AIS)患者早期睡眠障碍特征,了解急性缺血性脑卒中患者早期睡眠障碍的类型,为临床干预提供依据.方法:选取住院的急性缺血性脑卒中患者96例,入院后3天内完成匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quali-ty Index,PSQI)评估患者睡眠情况,根据PSQI量表评分标准及结果分为睡眠障碍组(n=60)和非睡眠障碍组(n=36),两组患者均进行整夜PSG监测,记录相关数据.比较两组患者一般资料及PSG相关睡眠参数.结果:①两组之间睡眠进程参数比较,睡眠障碍组的总睡眠时间及REM潜伏期均低于非睡眠障碍组(t=7.168,7.658;P<0.05),而睡眠潜伏期明显高于非睡眠障碍组(t=-7.161,P<0.05);②两组之间睡眠质量参数比较,睡眠障碍组的觉醒次数及觉醒指数均高于非睡眠障碍组(t=-11.728,-7.766;P<0.05),而睡眠效率明显低于非睡眠障碍组(t=5.684,P<0.05);③两组之间睡眠结构参数比较,睡眠障碍组的N1期及N2期均高于非睡眠障碍组(t=-7.147,-8.485;P<0.05),而REM期及N3期低于非睡眠障碍组(t=9.624,9.842;P<0.05);④两组之间睡眠呼吸事件比较,睡眠障碍组的AHI、仰卧位AHI、低通气指数均高于非睡眠障碍组(t=-7.618,-10.086,-11.185;P<0.05),而LSaO2低于非睡眠障碍组(t=8.098,P<0.05).结论:急性缺血性脑卒中患者早期存在明显的睡眠进程异常、睡眠质量下降、睡眠结构紊乱及睡眠呼吸事件方面的异常.应用PSG正确评估急性缺血性脑卒中患者早期睡眠障碍的类型并进行干预,有助于改善患者的预后.

Objective:To objectively analyze the characteristics of early sleep disorders in patients with Acute ische-mic stroke(AIS)by using polysomnography(PSG)and to learn about the types of early sleep disorders from patients with AIS,so as to provide basis for clinical intervention.Methods:A total of 96 patients with acute ischemic stroke were selected and assessed by Pittsburgh Sleep Quality Index(PSQI)within 3 days after admission.According to PSQI scale scoring criteria and results,patients were divided into sleep disorder group(n=60)and non-sleep disor-der group(n=36).Patients in both groups underwent overnight PSG monitoring and recorded relevant data.General data and PSG related sleep parameters were compared between the two groups.Results:①Compared with the param-eters of sleep process between the two groups,the total sleep time and REM latency of the sleep disorder group were lower than those of the non-sleep disorder group(t=7.168,7.658;P<0.05),while the sleep latency was signifi-cantly higher than that of the non-sleep disorder group(t=-7.161,P<0.05).② Compared with the sleep quality parameters between the two groups,the wake frequency and wake index of the sleep disorder group were higher than those of the non-sleep disorder group(t=-11.728,-7.766;P<0.05),while the sleep efficiency was significantly lower than that of the non-sleep disorder group(t=5.684,P<0.05).③Compared with the sleep structure parame-ters between the two groups,N1 and N2 in the sleep disorder group were higher than those in the non-sleep disor-der group(t=-7.147,-8.485;P<0.05),while REM and N3 were lower than those in the non-sleep disorder group(t=9.624,9.842;P<0.05).④Compared with sleep breathing events between the two groups,AHI,supine AHI and hypopnea index in the sleep disorder group were higher than those in the non-sleep disorder group(t=-7.618,-10.086,-11.185;P<0.05).LSaO2 was lower than that in the non-sleep disorder group(t=8.098,P<0.05).Con-clusion:Patients with acute ischemic stroke exhibit significant abnormalities in sleep progression,decreased sleep quality,disrupted sleep structure,and abnormal sleep respiratory events in the early stages.Applying PSG to correct-ly evaluate the types of early sleep disorders in patients with acute ischemic stroke and intervening can help improve their prognosis.
